Summary:
The Case Manager Acts As A Patient Advocate To Clients
Responsibilities:
  • 1. Applies Review Criteria To Determine Medical Necessity For Admission And Continued Stay
  • 2. Provides Clinically-Based Case Management, Discharge Planning, And Care Coordination To Facilitate The Delivery Of Cost Effective Quality Healthcare And Assist In The Identification Of Appropriate Utilization Of Resources Across The Continuum Of Care
  • 3. Meets Performance Expectations For Customer Service, Teamwork, Resource Utilization, And Staff And Self Development As Outlined In Performance Review.
  • 4. Performs Other Duties As Assigned Or Directed To Ensure Smooth Operation Of The Department/Unit.
    Other information:

    EDUCATION:
    Registered Nurse Graduated From An Accredited School Of Nursing With A License To Practice Professional Nursing In Connecticut. Bachelors Degree Required With Equivalent Education And Experience Considered. Master?S Degree Preferred.
    EXPERIENCE:
    Health Care Financing, Community And Organizational Resources, Patient Care Processes, And Data Analysis. Working Knowledge Of Healthcare Insurance And Financial Systems.
    LICENSURE:
    Current Ct R.N. License
    SPECIAL_SKILLS:
    Personal Computer Skills Required.
