Summary:
A registered nurse who provides ongoing assessment, planning, intervention, treatment and evaluation for REACH patients seeking care for psychiatric/mental health/behavioral problems and/or substance abuse. Job responsibilities include the following. Other duties may be assigned.

Responsibilities:
  • 1. Clinically assess REACH patients who present with complaints related to Psychiatric/Mental Health/Behavioral concerns or substance abuse.
  • 1.1 Demonstrates advance skill in interviewing, observing and collecting healthcare information and assessment on admission.
  • 1.2 Develops and implements preliminary treatment plan according to assessment and with guidance from physician.
  • 1.3 Completes assessment and implementation of preliminary plan.
  • 1.4 Completes documentation in a precise and timely fashion.
  • 2. Collaborates withMulti-disciplinary treatment team to develop treatment plans for patients.
  • 2.1 Performs psychiatric evaluation and diagnosis based on evaluation.
  • 2.2 Performs emergency consultation and treatment of patients.
  • 2.3 Prescribes psychotrophic medication and monitors patient-s response to medications through documentation.
  • 2.4 Consults with physician with regard to status of patients.
  • 3. Provides communication and coordination of treatment plans using clear and concise documentation on the patient''s medical record.
  • 3.1 Assures patient''s rights and safety are maintained.
  • 3.2 Documents all orders according to policy and procedure on physician order sheets and gets co-signatures in a timely fashion.
  • 3.3 Orders appropriate ancillary test.
  • 3.4 Maintains all privileges for APRN status including prescription privileges as per DEA regulations.
  • 3.5 Prescribes and administers medications following hospital policy and procedures.
  • 3.6 Participates in group tx planning and group therapy.
  • 3.7 Implements individual tx as needed.
  • 4. Obtains pre-certification and authorization for all patients.
  • 4.1 Appropriately identifies patients insurance status and obtains accurate authorization prior to admissions.
  • 4.2 Develops appropriate discharge planning process for patients.
  • 4.3 Follow EMTALA guidelines for safe and appropriate transfers when necessary.

    EDUCATION:

    Registered Nurse with Master''s Degree in Psychiatric/Mental Health Nursing.

    EXPERIENCE:

    Minimum 3 to 5 years clinical experience related to Psychiatric/Mental Health nursing.

    LICENSURE:

    Certified in Psychiatric/Mental Health nursing as clinical specialist

    APRN licensure.

    Certified as a Licensed Independent Practitioner (LIP) in the State of Connecticut.
