Summary:
A member of the psychiatric Multidisciplinary Team who is responsible for the care of assigned patients and assists with the Intensive Outpatient care under the direct supervision of a licensed individual. Essential duties and responsibilities include the following. Other duties may be assigned.
Responsibilities:
  • 1. Collaborates with the clinical team in the application of principles of patient assessment.
  • 1.1 Demonstrates basic skills in collecting data by interviewing patient and identifying overt and covert problems as observed by supervisor.
  • 1.2 Makes pertinent observations and documents same in patient record as observed by supervisor.
  • 1.3 Gathers demographic data when admitting a patient as recorded in patient record and as observed by supervisor.
  • 1.4 Gathers information necessary (from patient/family) in order to complete psychiatric intake as observed by supervisor.
  • 1.5 Accurately identifies patient problems, communicates and documents in accordance with MTP as evidenced by random review of nursing notes by supervisor.
  • 1.6 Identifies expected outcomes of interventions accurately and documents same based on feedback of Registered Nurse and random review of documentation as observed by supervisor.
  • 1.7 Identifies and communicates changes in patient''s condition to resource nurse and documents as noted by random review of nursing notes by supervisor.
  • 2. Collaborates with the Clinical Team in the application of principles for planning patient care.
  • 2.1 Collaborates with team in problem identification for nursing care plans as observed by supervisor.
  • 2.2 Gives verbal input into patients nursing plan of care in collaboration with team as observed by supervisor.
  • 2.3 Functions as a member of interdisciplinary treatment team by verbalizing observed changes in patients behavior and patients response to treatment and assists with comprehensive treatment planning as observed by supervisor.
  • 2.4 Collaborates with team in planning patient care and setting realistic outcomes.
  • 2.5 Collaborates with team in discharge planning as observed by supervisor and feedback from RN.
  • 2.6 Assists Psych. Tech. I in planning patient care as required as observed by supervisor.
  • 3. Works with the clinical team in the application of principles of implementation of patient care.
  • 3.1 Bases care on specific identified patient needs as observed by supervisor.
  • 3.2 Participates in leading and co-leading programmed milieu groups activities under supervision of program coordinator as observed by supervisor.
  • 3.3 Complies with policies and procedures in giving patient care as observed by supervisor.
  • 3.4 Organizes work for optimal use of time as evidenced by observation of supervisor.
  • 3.5 Demonstrates correct interventions specific to psychiatry as observed by supervisor.
  • 3.6 Demonstrates knowledge of CPI techniques and applies them correctly
  • 4. Works with the Clinical Team in the application of principles of evaluation of patient care.
  • 4.1 Reviews patient condition with team against expected outcomes as observed by supervisor.
  • 4.2 Measures patient responses to interventions based on expected outcomes as observed by supervisor.
  • 4.3 Evaluates results of counselling and teaching with team and documents same as observed by supervisor.
  • 4.4 Participates in process and retrospective Quality Assurance activities as evidenced by supervisory review of documentation.
  • 5. Provides Clerical support - including transportation coordination - to Adolescent and Medication Management program as needed.
  • 6. Documentation is accurate, timely and legible.
  • 6.1 Documentation is accurate, timely and legible.
  • 6.2 Maintains appropriate patient records as mandated by department policies.
    Documents evaluations, assessments, consultations, activities and treatment planning and other relevant information as per policy and in compliance with departmental and J.C.A.H.O. standards as indicated by quarterly random review of charts. Collaborates with clinical team in providing case management services including utilization reviews and discharge planning
  • 7. Is knowledgeable in the policy and procedures of seclusions /restraints for the Child Program. Is certified in CPI and maintains credentialing as required.
  • 7.1 Meets yearly competencies for application of seclusion protocol. Is certified in CPI and maintains certification. Other information: EDUCATION:
    Bachelors degree in psychology or related field,
    EXPERIENCE:
    Minimum of 2 years current clinical mental health experience.
    LICENSURE:
    Certified in CPI
    SPECIAL_SKILLS:
    Bachelors degree in Psychology; Or demonstrated professional growth as evidenced by attendance at conferences, educational reading, and supervision of senior clinicians.
