Summary:
Provides Social Work services to the hospital''s patients and their families; services are designed to address psycho-social and environmental factors which impact on the functioning and/or recovery of the patients and families. Essential duties and responsibilities include the following. Other duties may be assigned.
Responsibilities:
  • 1. Provides clinical social work services which may include crisis intervention, advocacy, bereavement counseling, information and referral, behavioral intervention to patients/families/significant others.
  • 1.1 Interviews patients, families, or significant others within one hour of referral as evidenced by documentation of patients'' charts.
  • 1.2 Gathers information regarding patient''s problems which impact on patient''s condition with 100% accuracy as measured by supervisory observation.
  • 1.3 Develops a psycho-social assessment of the impact of the identified problems in the patient in accordance with accepted professional and hospital standards as measured by supervisory observation.
  • 1.4 Develops plan for social work intervention in response to assessed needs and in cooperation with patient/family/significant others in accordance with accepted professional and hospital standards as measured by supervisory observation and feedback.
  • 1.5 Implements plan in accordance with accepted professional and hospital standards as measured by supervisory observation and feedback.
  • 1.6 Demonstrates and maintains current knowledge and skill in providing appropriate care for geriatric patients as observed by supervisor and as indicated by feedback from staf
  • 1.7 Demonstrates and maintains current knowledge and skill in providing appropriate care for adult patients as observed by supervisor and as indicated by feedback from staff.
  • 1.8 Demonstrates and maintains current knowledge and skill in providing appropriate care for young adult patients as observed by supervisor and as indicated by feedback from staff.
  • 1.9 Demonstrates and maintains current knowledge and skill in providing appropriate care for pediatric patients as observed by supervisor and as indicated by feedback from staff.
  • 1.10 Demonstrates and maintains current knowledge to address cultural issues as observed by supervisor and as indicated by feedback.
  • 2. Maintains effective, open communication with all members of the health care team and other hospital staff.
  • 2.1 Functions as an active, contributing member of an inter-disciplinary team as measured by feedback from health care team.
  • 2.2 Demonstrates collaboration with other disciplines in service provision as measured by supervisory observation and feedback.
  • 2.3 Interacts with co-workers, physicians, and other team members in a positive, cooperative manner as measured by supervisory observation and feedback.
  • 3. Maintains appropriate patient records as mandated by department policies.
  • 3.1 Completes written report of evaluation and treatment plan for patients'' chart within two hours of initial consultation as indicated by review of charts and supervisory observation.
  • 3.2 Documents activities and treatment planning in compliance with departmental and J.C.A.H.O. standards 100% of the time as indicated by quarterly random review of charts.
  • 4. Develops and maintains positive working relationships with community agencies/facilities and service providers to enhance the availability of resources and to facilitate patient access.
  • 4.1 Demonstrates knowledge of community resources and utilizes them effectively as observed by supervisor.
  • 4.2 Maintains necessary written and verbal communications with community agencies and facilities as measured by supervisory observation and feedback.
  • 5. Demonstrates knowledge of department/hospital programs policies and procedures.
  • 5.1 Identifies problem areas or gaps in services in hospital and department in accordance with professional standards measured by supervisory observation.
  • 5.2 Makes effective recommendations for developing solutions to identified problems or service gaps as measured by supervisory observation.

    EDUCATION:
    Master''s Degree in Social Work

    EXPERIENCE:
    Experience in health care setting and crisis intervention skills.

    LICENSURE:
    LCSW or eligible required.

    PHYSICAL_DEMAND:
    While performing the duties of this job, the employee is regularly required to stand, walk, and talk and/or hear. The employee is occasionally required to sit. The noise level in the work environment is usually moderate.
