Summary:
Working collaboratively as a member of a multidisciplinary treatment team, the Milieu Counselor is assigned to and works under the direction of a Registered Nurse, participating in carrying out nursing interventions to provide patients with opportunities for learning and adaptive ways of coping. The Milieu Counselor works collaboratively with the licensed staff on the ongoing development of the milieu and activities that promote quality patient outcomes.
Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based on the 4 principles of PFCC: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence by creating a great "First Impression" by demonstrating exemplary customer service skills for all customer groups including patients, families/friends, physicians, staff and support department personnel.

Responsibilities:
  • 1. Under the direction of the Registered Nurse, provides individualized patient care to insure the safety and dignity of the patient.
  • 1.1 Collaborates with members of multidisciplinary team offering suggestions and recommendations on addressing patient/family needs.
  • 1.2 Follows the patient care plan preserving patients'' safety, dignity and autonomy.
  • 1.3 Assists the RN in obtaining specimens within 24 hours of orders; obtains and records accurate I&O data and accurately records behavioral scale observations, while preserving patient''s safety, dignity and autonomy.
  • 1.4 Teaches and assists patients with activities of daily living while preserving patient safety, dignity and autonomy.
  • 1.5 Collaborates with the Nurse to assist in the development and ongoing evaluation of the patient''s plan of care and development of discharge plan.
  • 1.6 Notifies the patient''s nurse of any changes in the patient''s physical/ psychiatric condition and requests assessment by the primary/associate hospital.
  • 1.7 Collaborates with the nurse to assist patient in undergoing testing and consultations in other areas of the hospital
  • 1.8 In concert with the nurse, develops alliances with the patient''s family; participates in teaching behavioral techniques consistent with the NCP and in collaboration with the Interdisciplinary Team.
  • 1.9 Performs vital signs as delegated by RN.
  • 1.10 Performs glucose monitoring and phlebotomy (if certified as delegated by RN).
  • 1.11 Documents patient care and action in the patient record , in a clear, conscise manner.
  • 2. Assists the nursing staff in maintaining a therapeutic milieu/environment to ensure the care and treatment of patients.
  • 2.1 Acts as a co-facilitator in select unit based therapeutic groups as identified by Nurse Manager.
  • 2.2 Engages in anticipatory planning and problem solving in collaboration with the Nurse and MC within the milieu/environment.
  • 2.3 Initiates patient activities and social interactions in individual and group situations consistent with the nursing plan of care.
  • 2.4 Provides environmental and emotional safety for each patient through consistent limit setting, physical holding and restraining, as well as monitoring activities and interactions of visitors, under direction of the Registered Nurse.
  • 2.5 Maintains a clean and safe patient environment.
  • 2.6 Collaborates with the MC, RN, OT, RT in co-facilitating activities and/or treatment oriented groups.
  • 2.7 Acts as a facilitator in select unit based, task oriented groups as identified by Nurse Manager.
  • 3. Participates in unit activities that promote a cohesive staff group.
  • 3.1 Participates in interdisciplinary staff meetings and if unable to attend, reads and signs minutes.
  • 3.2 Participates in and attends nursing staff meetings as scheduled and if unable to attend, reads and signs minutes.
  • 3.3 Participates in unit based activities that have a positive impact on unit practice (e.g., collaborates with licensed staff on inservice presentations, policy development task force, etc.)
  • 3.4 Maintains all mandatory continuing education classes and attends three (3) hours of inservice classes yearly. If attends conferences outside of unit will bring information back to be shared.
  • 3.5 Attends nursing supervision as scheduled and seeks supervision from RN, Psychiatric Service SLE, Nurse Manager, as appropriate.

    EDUCATION:

    Associates degree required and Bachelor degree preferred

    EXPERIENCE:

    Minimum two (2) years prior experience in Psychology working with specialized populations such as emotionally disturbed children and families.

    SPECIAL_SKILLS:

    Ability to work collaboratively in an interdisciplinary team to communicate and effectively deal with complex effectively charged situations and possess group facilitation and crisis management skills.
