Summary:
As a core member of the Care delivery team, the Clinical Nurse practices as the patients nurse and collaborates with the team to facilitate the coordination of patient care at the unit level. Through utilization of the nursing process, the clinical nurse diagnoses and treats patient and family responses to actual or potential health problems. The Clinical Nurse is accountable to the patient, family, peers, and Manager.

Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based on the 4 principles of PFCC: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence by creating a great "First Impression" by demonstrating exemplary customer service skills for all customer groups including patients, families/friends, physicians, staff and support department personnel.

Responsibilities:
  • 1. Nursing Process
  • 1.1 The Clinical Nurse II utilizes the nursing process of assessment, planning, intervention and evaluation while caring for our patients maintaining confidentiality and dignity. The Clinical Nurse II provides safe, effective evidenced based care tailored to the individual needs of the patient and family. As a core member of the care delivery team, the Clinical Nurse II partners with the team to facilitate the coordination of patient care at the unit level utilizing the principles of patient/family centered care.
  • 1.2 The clinical nurse uses a 5 step process to deliver high quality comprehensive and individualized care incorporating:

    1.Patient assessment
    2.Problem identification
    3.Establishment of desired outcomes
    4.Implementation of interventions
    5.Evaluation of care with necessary revisions.
  • 2. Clinician Patient Relationship
  • 2.1 Demonstrates a holistic and caring approach throughout the nurse patient relationship.
  • 2.2 Provides safe, effective evidenced base care tailored to the individual needs of the patient and family.

  • 5.9 Identifies own learning needs in collaboration with the Patient Services Manager/designee.

  • 5.11 Delivers excellence in patient care through compliance with all patient safety and regulatory initiatives.
  • 5.12 Each CN II is expected to have at least two positive impacts each year -which will be included in the review.
  • 6. Leadership
  • 6.1 Evidence of self development and positively influencing practice environment on a consistent basis.
  • 6.2 Contributes to behavioral and clinical changes in their practice setting as an equal member of the health care team within own role and responsibilities.
  • 6.3 Participates in the assessment, development and presentation of programs to address clinical
    and/or educational needs of staff members in collaboration with the clinical manager, CNS,
    care coordinator, or other appropriate health care team members.
  • 6.4 Responsible for being current with staff meeting minutes.
  • 6.5 Communicates in a calm professional manner with patients, families, colleagues, members of the care team, and other hospital personnel, accepts and elicits feedback, and works toward resolving problems and clarifying issues and concerns.
  • 6.6 Uses resources in a fiscally sound manner.

    EDUCATION:

    Graduation from an accredited School of Nursing., Licensed and a currently registered nurse in the State of Connecticut. All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire.

    EXPERIENCE:

    After completion of at least one (1) year of practice consistent with the four domains identified in the RCAP CMP a CN II may be eligible to apply for advancement to CN III., An experienced RN who was hired at the CNII level may be eligible to apply for CNIII advancement after 6 months., All RNs are expected to advance to their optimal level of functioning through promotion., Please see RCAP CMP for further details,

    Special Skills: Excellent communications, interpersonal, coordinative, and organizational skills.
    ACCOUNTABILITY:

    Provides nursing care to an individual or group of patients; recognizes changes in patients condition and reports such changes to Physician and/or Patient Services/designee., Utilizes and manages material resources in a costeeffective manner.
    COMPLEXITY:

    Plans, develops and implements patient care plans; evaluates the effectiveness of existing patient care; evaluates the effectiveness of existing patient care plan and modifies plan of care to meet patient needs.
    PHYSICAL_DEMAND:

    Requires prolonged standing; walking; use of sight, hearing, and touch; moving/lifting patients of varying weights.
