Responsibilities: 1. Resolves drug distribution problems assuring accurate and timely dose delivery to all patients 1.1 Responds to telephone requests for medication in appropriate fashion accommodating callers request or routing call to appropriate person. 1.2 Understands patient specific drug therapy management plans and therapy endpoints. 1.3 In a timely fashion, reviews and evaluates all medication orders to assure safe, accurate, cost effective drug use. 2. Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans. 2.1 Demonstrates knowledge of basic pharmacokinetic principles. 2.2 Performs pharmacokinetics calculations. 2.3 Recommends drug therapy changes to prescribers, and/or prospectively initiates appropriate dosage regimens. 3. Problem Identification & Resolution 3.1 Communicates and documents problem detection, relevant findings, recommendations, and necessary follow-up. 3.2 Utilizes pharmacist follow-up column to communicate issues from shift to shift. 3.3 Identifies recurring drug-related problems and suggests a plan for improvement. 4. Provides drug information to nurses, MDs, and other health care professionals 4.1 Serves as primary source of drug information to physicians, nurses, and other health care professionals 4.2 Utilizes a systematic approach in order to research and respond to drug information requests 5. Educational Activities 5.1 Participates in and contributes to educational forums within the department (written or verbal). 5.2 Completes department education and competencies in a timely manner in order to satisfy regulatory requirements 6. Documents medication use variances (MUVs) and Adverse Drug Reactions (ADRs). 6.1 Provides vialble solutions to prevent recurrence of MUVs 6.2 Encourages other caregivers to document MUVs and ADRs 7. Regulatory and Safety Quality Compliance 7.1 Effectively demonstrates the principles and conduct consistent with a High Reliability Organization.
Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.
Two years experience in an acute care hospital pharmacy is preferred for this position.
Licensed to practice pharmacy in Connecticut (or eligible for reciprocation)
This individual should demonstrate knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution, and formulary management. Excellent communication skills and the ability to work with others are required.
The position requires the ability to effectively communicate, both written and verbal. This individual requires a high degree of planning and exercising discretion in making therapeutic and technical decisions. Must apply new ideas, knowledge and skill gained from experience and education to current situations; and should possess a knowledge of current standards of pharmacy practice.
