Responsible for coordinating clinical aspects of multi-organ transplantation as it applies to the transplant recipient. Assists in the development and growth of an individualized program for patients referred for possible transplantation. Responsibilities include coordination of the pre-transplant evaluation, registration and listing of potential transplant recipients, and post transplant care. As an integral member of an interdisciplinary team, coordinates and performs various other clinical, educational, administrative, and technical responsibilities related to clinical transplantation. The Clinical Transplant Coordinator provides 24 hour on call coverage for YNHTC including patient calls, organ offers and transplant surgery scheduling.

  • 1. Clinical Practice
  • 1.1 Coordinates all recipient and potential donor aspects of transplantation including initial candidate referral, pre-transplant evaluation, candidate selection, wait listing, transplant admission, and post transplant follow-up care.
  • 1.2 Patient care responsibilites will be performed consistent with age specific criteria.
  • 1.3 Participates and organizes the evaluation of patients who are referred as potential transplant candidates.
  • 1.4 Coordinates the organization of cadaver and living donor transplantation
  • 1.5 Coordinates pre-transplant evaluation of potential recipients and potential living donors; including screening and candidate selection
  • 1.6 Interviews patients and reviews medical history to screen potential recipients for acceptable criteria for transplant candidacy.
  • 1.7 Coordinates, in collaboration with the histo-compatibility and immune evaluation laboratory staff, the tissues typing activities of potential transplant recipients.
  • 1.8 Coordinates activities involving clerical and patient representative personnel, such as for scheduling procedures, assuring the patients are placed on the transplant waiting list, and scheduling admissions to the hospital as necessary.
  • 1.9 Assures timely and organized pre-transplant intake visits, efficient and organized pre-transplant evaluation and placement on the transplant waiting list.
  • 1.10 Maintains contact with patients on the transplant waiting list from time of listing until a donor organ becomes available. This includes assuring that medical problems potentially complicating or preventing transplantation have not arisen.
  • 1.11 Coordinates operating room activities related to the transplant, especially for non-renal (eg. Liver and pancreas) transplant procedures
  • 1.12 Coordinates post transplant management activities, including outpatient visits to transplant clinical as well as visits to the patients'' private physicians.
  • 1.13 Assures effective communication regarding patient issues with the various members of the transplant team including the medical staff, data coordinator, social services staff, financial service staff, and other relevant support staff.
  • 2. Patient Satisfaction and Service Excellence
  • 2.1 Participates in the development of patient relations standards reflecting excellence consistent with hospital policy for internal/external customers in collaboration with YNHH and YNHTC leadership
  • 2.2 Maintaining accountability for adhering to patient relation and service excellence standards
  • 2.3 Ensures that the confidentiality of patient and appropriate YNHTC data is maintained
  • 2.4 Serves as a central advocate for prospective transplant recipients and donors (as appropriate) through the transplant evaluation process
  • 2.5 Maintains contact woth referring physicians and comminuty healthcare providers to assure effective communication
  • 2.6 Serves as a liason between referring physicians, dialysis units and the YNHTC to facilitate patient evaluations and post transplant follow up care and to provide timely feedback.
  • 3. Performance Improvement
  • 3.1 Monitor clinical outcomes and system issues related to quality of care delivery to patients within YNHTC
  • 3.2 Participate as a clinical consultant to establish YNHH policies for multi-organ transplantation
  • 3.3 In collaboration with the data coordinator, assist with the preparation and maintenance of clinical and statistical reports for the transplant program, regional and national networks to document evaluation and outcome of transplant procedures.
  • 4. Organizational Leadership
  • 4.1 Interfaces with multiple hospital areas in coordinating patient education, psycho-social services and various other aspects of the transplant clinic experience.
  • 4.2 Provides liaison support for the inpatient nursing staff regarding transplant patient issues, particularly with regard to post transplant patient education.
  • 4.3 Conducts professional education and in-service training programs for hospital and other medical personnel to develop commitment to and understanding of transplantation.
  • 4.4 Acts as an advisor on ethical and social issues in transplantation to promote continued interest and commitment to the program.

    Other information:


    BSN /or appropriate certification/licensure. Masters Degree is also desirable..


    Two (2) to five (5) years clinical patient care in an area relevant to transplantation. Actual experience in organ transplantation is highly desirable and preferred


    Registered Nurse or Physician''s Assistant recommended
    Certified Clinical Transplant Coordinator (CCTC) recommended within 2 years of starting position


    Highly developed clinical, communication, and interpersonal skills are essential.

    Coordinates all aspects of multi-organ transplant care. The Transplant Coordinator ensures effective coordination of initial referral for evaluation; pre-transplant evaluation, transplant surgeries, and post transplant follow up. Serves as an advocate and clinical consultant for patients, families, Hospital and medical personnel involved with multi-organ transplant patient care. Performs a variety of educational, administrative and clinical functions related to organ transplantation..

    Advanced clinical skills, clinical decision making, clinical triaging, and administrative decision making are each important aspects of this position. Similarly, highly developed communication and interpersonal skills are essential. In-service training to hospital and medical staff, including instructional materials and consultation to patients and their families are also expected. This position is responsible for coordinating the highly complex and detailed events essential to the success of the transplantation process.
