Summary: The primary function of Advanced Nurse Practitioner (APRN/NP) working for NEMG YNH Geriatric Services is to provide comprehensive care to geriatric patients in primary care, home care and long & short term settings in collaboration with attending physicians. Each APRN works with the appropriate M.D. to provide outstanding and coordinated medical care. The APRN also participates in education of students and Geriatric Fellows. Exemplifies behaviors reflecting Service Excellence and support of the Strategic Dimensions by modeling the Core Success Factors as described in the Performance Review. Responsibilities: \u2022 1. Clinical Practice: o 1.1 Coordinates patient care plan with attending M.D. and nursing staff. Communicates with other providers as needed. o 1.2 Communicates patient change in status information and test results to Attending Physicians. o 1.3 Completes all documentation and billing in the EPIC electronic medical record. Charting and billing are done each day for all patients seen that day. o 1.4 Identifies self to assigned patients and their families as APRN. Wears ID badge above waist at all times. o 1.5 Serves as a liaison and interacts with nursing staff, rehabilitation staff, and other ancillary services to ensure timely and comprehensive care for all patients. o 1.6 Serves as a liaison to patients and their families. o 1.7 Educates patients and families about plan of care. o 1.8 Promotes the dissemination of the newest clinical evidence in literature to improve patient care. \u2022 2. Education o 2.1 Identifies and provides information needs of team members, patients and families. o 2.2 Provides related teaching and educational materials to students and geriatric fellows. o 2.3 Acts as a clinical resource for residents and fellows, and as a role model/preceptor for PA students. Maximizes opportunities in the clinical setting for informal education of staff, patients, and families. o 2.4 Actively researches information on patient medical conditions and shares information with other team members and students. o 2.5 Attends Geriatric Grand Rounds at the Adler Conference Room every other Tuesday at 4pm . o 2.6 Participates in a level of continuing medical education- CME activity annually that meets all medical staff reappointment criteria and improves the quality of care provided by the P.C. o 2.7 Responsibly utilizes annual allotment of CME funds/days throughout the fiscal year. o 2.8 Participates in annual Risk Management meetings through Yale-New Haven Hospital since liability insurance coverage is provided through the Yale plan. This is not mandatory but is expected. o 2.9 Maintains privileges at Yale-New Haven Hospital. \u2022 3. Quality Improvement/Customer Satisfaction o 3.1 Participates in team based quality improvement and patient/customer satisfaction initiatives. o 3.2 Responds to patient complaints and reports patient satisfaction concerns to Coordinator, Associate Director, or Medical Director so that appropriate action may be taken before patient discharge. o 3.3 Ensures medical record documentation meets YNHHS, State and Federal guidelines, clearly documenting patient diagnosis and medical information required for legal and billing purposes. \u2022 4. Patient Safety o 4.1 Practices medicine in a manner to maximize patient safety. o 4.2 Immediately reports any concerns about patient safety to Medical Director, Associate Director. or Coordinator. o 4.3 Follows all local affiliate patient safety policies and procedures for healthcare providers. o 4.4 As a healthcare provider, is responsible for reporting adverse or potential adverse events related to patient care. \u2022 5. Operations o 5.1 Follows all NEMG YNH Geriatric Services policies and procedures. o 5.2 Responds to all calls in a timely manner. o 5.3 Notifies secretary and Coordinator if cell phone is lost, stolen or malfunctioning. o 5.4 Completes all HealthStream mandatory training requirements related to HIPAA, Emergency Management, patient safety, or as assigned. Checks for new courses at the first of each month. o 5.5 Maintains current licensure and certifications by submitting renewal immediately upon receipt. o 5.6 Bills for all medical services provided. o 5.7 Attends all NEMG YNH Geriatric Services clinical staff meetings as scheduled Notifies Coordinator if unable to attend. o 5.8 Dresses and behaves professionally at all times. Adhering to the Appearance policy and Code of Conduct policy. o 5.9 Works the number of hours necessary to complete all work on a daily basis and to offer support to others. o 5.10 Holds a valid Connecticut medical license, Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification. EDUCATION: Graduation from an accredited APRN program required. Licensed APRN by the State of CT. Holds a valid Connecticut medical License. Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification. EXPERIENCE: One (1) to two (2) years caring for internal medicine or geriatric patients preferred. SPECIAL_SKILLS: Excellent clinical, interpersonal and organizational skills. Ability to work independently with minimal supervision. ACCOUNTABILITY: Accountable to conduct accurate and appropriate evaluation, care, and coordination of care of nursing home/geriatric patients. COMPLEXITY: Evaluates, manages, and coordinates care of geriatric patients under the supervision or in collaboration with the attending physicians. May assist attending physicians in all aspects of patient care, including interactions with other services involved in the patient\u2019s care. PHYSICAL_DEMAND: MMR, Tdap and influenza vaccination are required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required.
