LICENSED CLINICAL SOCIAL WORKER OUTPATIENT BEHAVIORAL HEALTH - ADOLESCENT AND ADULT TRUMBULL, CT

Summary: The Clinical Social Worker I is a licensed experienced professional who is the clinical expert in the assessment and treatment of pediatric, adolescent, adult and dual diagnosed patients/families in an outpatient setting. As the clinical expert the scope of practice includes but is not limited to diagnostic assessment, counseling, psychotherapy, brief treatment, behavior modification, mental health consultation, teaching/mentoring, projects/task force, inservice/lecture, article/paper, significant clinical practice enhancement, participation in research, and mentor/supervise students. The clinical social worker will report to the Director of Outpatient Behavioral Health and work collaborateively with members of the health care teams throughout the organization. Responsibilities:
1. Licensed Social Work Clinical Practice
1.1 Develop a comprehensive biopsychosocial assessment responsive to the Adolescent, Adult and Dual Diagnosed patients/families/staff needs and concerns.
1.2 All patient care responsibilities will be performed consistent with age-specific criteria.
1.3 Expert diagnostic biopsychosocial assessment for disability and impairment
1.4 Develop a treatment intervention plan in collaboration with the multi disciplinary team responsive to patient/family concerns, problems and clinical impressions.
1.5 Initiates and maintains contact with managed care representatives as needed to obtain pre-certification and insure reimbursement for services as appropriate.
1.6 Develop an effective working relationship with patient/family to engage, collaborate, advocate and problem solve in order to enhance their functioning.
1.7 Utilize as a treatment modality, individual, couples, family and/or group psychotherapy and/or crisis intervention.
1.8 Provide information, consultation and professional support to interdisciplinary team members and colleagues within the Hospital and with community providers.
1.9 Provide education of staff related to an individual and/or family''s psychosocial needs.
1.10 Demonstrates initiative and leadership in developing services which respond to patient care demands.
1.11 Identify practice based on continuous quality improvement.
1.12 Maintain an up-to-date, accurate and appropriate record of social work activities in patients'' medical chart, which does not violate confidentiality.
1.13 May provide expert teaching, mentoring and participate in in-service/lectures.
1.14 May participate in projects/task force resulting in significant practice enhancement.
1.15 May author article/paper or participate in research project.

3.1 Accountable for following hospital policy and procedures, NASW Code of ethics, and any other Hospital, governmental and/or regulatory policies, rules or regulations.
Other information: EDUCATION: Master''s Degree from an accredited school of social work. EXPERIENCE: Qualifying social work experience for licensure consists of a minimum of 3,000 hours of post-masters social work. Experience working with children, adolescent and adults individually and in groups preferred. LICENSURE: Licensed Clinical Social Worker (LCSW) from the State of Connecticut, or those holding licenses from other states, will need an ''endorsement'' and meet the State of Connecticut license requirements., SPECIAL_SKILLS: Ability to perform independently and provide clinical practice in a health care setting. Computer experience and knowledge of word processing, spreadsheet, and data entry systems. ACCOUNTABILITY: Accountable for following Hospital policy and procedures, NASW Code of Ethics, and any other governmental and/or regulatory policies; will meet expectations of growth goals set forth by the performance appraisal document. COMPLEXITY: Participate in Quality Improvement activities and departmental/hospital committees. PHYSICAL_DEMAND: MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap and influenza vaccination are strongly encouraged.
