Summary:
The primary function of the Physician employed by Northeast Medical Group - YNH Geriatric Services is to provide high quality medical care to older patients in affiliated nursing homes, rest homes and retirement communities. The physician coordinates care with other clinicians (ie: specialists, community pcp''s) and with Yale-New Haven Hospital for admissions.
Responsibilities:
  • 1. Clinical Practice:
  • 1.1 As patients are assigned in the nursing home setting or switch to one of the primary care practices the M.D. is to admit the patient to the practice in a timely manner and to ensure safe, high quality and efficient care that is coordinated with other clinical professionals.
  • 1.2 For new nursing home admissions M.D. follows state and federal guidelines for obtaining comprehensive health histories and performing complete physical examinations within 48 hours of arrival at the facility. The Physician ensures that admission orders for medications, rehabilitation therapy, laboratory and diagnostic imaging tests are written.
  • 1.3 Develops a plan of care for each nursing home patient and works closely with facility staff for the best possible outcome.
  • 1.4 Following state, federal and facility guidelines, nursing home patients are seen every thirty days and have an annual exam one time per year. M.D. utilizes schedule provided to assure that patients are seen a timely manner. Patients may also be seen for problem or follow up visits between the monthly exams.
  • 1.5 Identifies self as MD and wears YNHH badge at all times.
  • 1.6 Serves as a liaison to patients and their families.
  • 1.7 Regularly communicates patient treatment plans to patients.
  • 1.8 For sub-acute patients the M.D. communicates patient treatment plans to patients- primary care physicians in the community. Alerts primary care physicians upon patient admission, and communicates with them regularly through discharge.
  • 1.9 If working at Arden House, M.D. is available to PA-s throughout the day.
  • 1.10 Communicates with nursing staff, consultants and other ancillary staff as appropriate.
  • 1.11 Works with social work to educate patients and families about transition needs. Completes patient discharge summaries and routes to secretary for copies to be mailed to community physicians.
  • 1.12 Completes all facility discharge paperwork in a timely manner.
  • 1.13 Primary care patients are seen and admitted to the Practice only after Medical Record is received and reviewed.
  • 1.14 Primary care patients are seen on a quarterly basis as is the Practice policy for providing comprehensive and safe care to older patients.
  • 1.15 Primary care physicians approve orders and medications only for Practice patients.
  • 1.16 Utilizes all office systems making recommendations for improvements when appropriate.
  • 1.17 Does all charting and billing in the Centricity Electronic Medical Record (CEMR). Charting and billing are done each day for all patients seen that day. Check out documents are routed to the biller daily.
  • 1.18 Signs and returns Rx and Certificate of Medical Necessity forms in a timely manner returning to secretary to distribute and maintain copies.
  • 1.19 Directly supervises medical and PA students, residents and geriatric fellows as assigned.
  • 1.20 Participates in on-call coverage for nights, weekends and holidays. Is responsible for making changes to the schedule with colleagues, when necessary, and notifies Coordinator.
  • 1.21 Assists in coordination of on-call protocols with each facility assigned. Works closely with Director of Nursing to assure adequate education is provided to nursing staff to streamline calls.
  • 1.22 Attends facility meetings as required for Medical Staff participation and, if Medical Director, for oversight of facility and pharmacy functions.
  • 1.23 Participates in the Health System payroll process including calling into the Kronos system at the start of each work day and calling out at the end of the day.
  • 1.24 Promotes the dissemination of the newest clinical evidence in literature to improve patient care.
  • 1.25 Offers assistance to other team members to assure proper coverage of all patients.
  • 1.26 Maintains privileges at Yale-New Haven Hospital.
  • 2. Education:
  • 2.1 Identifies information needs of team members, patients and families.
  • 2.2 Regularly provides related teaching and education to facility nurses and PA-s (if at Arden House).
  • 2.3 Provides teaching with structure to students from the Yale University School of Medicine Physician Associate Program, Medical Students and Geriatric Fellows as assigned.
  • 2.4 Acts as a clinical resource for students, residents and fellows, and as a role model/preceptor for PA-s. Maximizes opportunities in the clinical setting for informal education of staff, patients, and families.
  • 2.5 Assists in orientation of new PA-s and Physicians hired by Northeast Medical Group - YNH Geriatric Services.
  • 2.6 Attends Geriatric Grand Rounds at the Adler Library every other Tuesday at 4pm.
  • 2.7 Makes presentations at Geriatric Grand Rounds as assigned.
  • 2.8 Attends appropriate Continuing Medical Education symposiums, conferences, etc. to maintain education of current medical practice as well as meeting the requirements for Yale-New Haven Hospital Medical Staff privileges.
  • 2.9 Responsibly utilizes annual allotment of CME funds throughout the fiscal year.
  • 2.10 Participates in a level of continuing medical education (CME) activity annually that meets all medical staff reappointment criteria and improves the quality of care provided by the Practice.
  • 2.11 Participates in mandatory annual Risk Management meetings through Yale-New Haven Hospital in order to participate in the Yale-New Haven Hospital liability insurance plan.
  • 3. Quality Improvement/Customer Satisfaction
  • 3.1 Participates in team-based quality improvement and patient/customer satisfaction initiatives.
  • 3.2 Responds to patient or family complaints and reports patient satisfaction concerns to Coordinator, Manager or Medical Director so that appropriate action may be taken.
  • 3.3 Assures that all medical record documentation meets Practice, facility, State and Federal guidelines, clearly documenting patient diagnosis and medical information required for legal and billing purposes.
  • 4. Patient Safety
  • 4.1 Practices medicine in a manner to maximize patient safety.
  • 4.2 Immediately reports any concerns about facility patient safety to Medical Director, Manager or Coordinator.
  • 4.3 Follows all Yale-New Haven Hospital patient safety policies and procedures for healthcare providers.
  • 4.4 As a healthcare provider, is responsible for reporting adverse or potential adverse events related to patient care.
  • 5. Operations
  • 5.1 Follows all Northeast Medical Group - YNH Geriatric Services policies and procedures.
  • 5.2 Responds to pages in a timely manner.
  • 5.3 Notifies secretary and Coordinator if pager is lost, stolen, or malfunctioning.
  • 5.4 Completes all HealthStream mandatory training requirements related to HIPAA, Emergency Management, patient safety and others as assigned checking for new course assignments on the first of each month.
  • 5.5 Assures maintenance of credentials without interruption by submitting renewals immediately upon receipt.
  • 5.6 Faxes all renewed license and registrations to Coordinator immediately upon receipt.
  • 5.7 Attends Northeast Medical Group - YNH Geriatric Services clinical staff meetings as scheduled approximately every 4 weeks throughout the year. Notifies Coordinator if unable to attend.
  • 5.8 Bills for all services provided each day on that day.
  • 5.9 Dresses and behaves professionally at all times.
  • 5.10 orks the number of hours necessary to complete all work on a daily basis and to offer support to others.
    Other information:

    EDUCATION:

    M.D. degree, full residency training and Board eligibility/certification in internal medicine mandatory. Internal Medicine Board certified with Added Qualifications in Geriatrics preferred. Must have attending medical staff privileges at Yale-New Haven Hospital, hold a valid Connecticut medical license, Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification.

    EXPERIENCE:

    Two years of experience in primary care or nursing home care and/or completion of fellowship in primary care or geriatric care.

    LICENSURE:

    Cirrent MD license

    SPECIAL_SKILLS:

    Excellent clinical, interpersonal and organizational skills. Ability to work independently.
    ACCOUNTABILITY:

    Accountable to conduct accurate and appropriate evaluation, care, and coordination of care for older patients.
    COMPLEXITY:

    Evaluates, manages, and coordinates care for geriatric patients in nursing home, rest home and primary care settings. In some settings works as part of the Physician-PA team. Works with Administrative staff on special projects and Practice expansion as assigned.
    PHYSICAL_DEMAND:

    MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids.
    Tdap and influenza vaccination are strongly encouraged.
