This position reports to the Director of Rehabiltation and The therapist will evaluate and implement a plan of treatment as appropriate and related to resident''s current medical diagnosis(s). The Therapist will perform care within the State and Federal Physical Therapy Practice Regulation.
1. Clinical 1 .1 Able to interpret physician referrals to perform basic clinical evaluations. 1 .2 All patient care responsibilities will be performed consistent with age-specific criteria. 1 .3 Demonstrates technical skill and accuracy in performing basic treatments. Develops population specific treatment programs which reflect compliance with established clinical protocols where appropriate, monitors patient responses and terminates treatments when appropriate. 1 .4 Sets measurable and appropriate goals which reflect patient/family involvement and seeks appropriate assistance. 1 .5 1 .6 Provides patient and family education. Initiates and maintains current records and reports concerning patient course, re-evaluates and updates goals, and plans as necessary, based on patient's response to treatment. 1 .7 1 .8 Uses equipment in a safe and effective manner. Reports malfunctioning or missing equipment to supervisor and takes corrective action to remove equipment from use. 1 .9 1 .10 Assumes responsibility for order of working areas. Maintains a high standard of professional accountability with respect to safety, efficiency, effectiveness, ethics and legal issues. 1 .11 1 .12 Demonstrates compliance with all hospital standards and regulations. 1 .13 Participates constructively in team conferences, rounds, clinics, and discharge planning, as appropriate. 1 .14 Utilizes assistant and support staff and volunteers appropriately in the delivery of patient care. 1 .15 Meets departmental standards for productivity. 1 .16 Discharges patients in compliance with cancellation/no show practices. 2. Educational 2 .1 Attends department in-services. 2 .2 Assists with orientation of staff as appropriate. 2 .3 Assists in orientation, education and guidance of support staff. 2 .4 Seeks opportunities to expand own knowledge of the profession and related fields. 2 .5 Meets required standards for documentation of attended in-services and CEU's when applicable. 3. Administration 3 .1 Complies with Grimes Center and Department Policies and Procedures. 3 .2 Participates in department Quality Assurance activities. 3 .3 Plans and organizes daily schedule optimally use time. 3 .4 Appropriately addresses patient coverage issues for scheduled time off. 3 .5 Carries patient load that reflects productivity standard of unit. Maintains documentation in accordance with hospital policy and regulatory requirements ( HCFA, JCAHO, etc.). 3 .6 4. Clinical Supervision 4 .1 Participates in regular meetings with supervisor and reports pertinent information on a timely basis. Appropriately delegates responsibility to Physical Therapy Assistants and maintains an appropriate level of clinical supervision. 4 .2 4 .3 Provides clinical supervision to students as assigned. 5. Human Relations Skills Demonstrates respect and regard for dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsive and courteous hospital environment. 5 .1 Initiates and maintains appropriate communication with patient, family, and other health care team members during the patient's course of treatment. 5 .2 5 .3 Cooperates with office associates to resolve issues raised (i.e., scheduling, documentation, etc.). Formulates and uses effective working relationships with all health care team members , patients, and others. 5 .4 5 .5 Treat all other members of the team with mutual respect.
Bachelor of Science in Physical Therapy required, masters preferred.
Recent graduate of Physical Therapy program
Current Physical Therapy State of Connecticut licensure.
1.The therapist is able to integrate the five elements of patient/client management:
•Examination, Evaluation,Diagnosis, Prognosis, Intervention
2.Will be able to identify appropriate tests and measures necessary to guide them in making the right decision for intervention.
3.Will be able to implement and modify treatment plans in accordance with the patient?s needs.
4.Will be able to identify wen a re-assessment or discharge should occur.
5.Will posses the skills necessary to determine need for intervention through a screen.
6.Will complete all clinical documentation clearly and in compliance with company, state, federal and intermediary guidelines.
