Summary:
The Nursing Assistant has the knowledge and ability to assist in the delivery of patient care as delegated by a registered professional nurse. Able to provide appropriate care to patients regardless of age, showing sensitivity toward individual needs. Willing to accept responsibility for individual job requirements and performance, able to contribute to effective unit operations.
Responsibilities:
  • 1. Assists the nurse in providing care to patients.
  • 1.1 Provides direct physical patient care following the accepted procedures and in accordance with the nursing assistant''s skills.
  • 1.2 Communicates complaints of pain, requests for pain medication or changes in body function to the Registered Nurse.
  • 1.3 Assures patients have clean water and personal items needed are within reach. Assures that the call light is within the patient''s reach. Participates in maintaining the cleanliness of the patient''s area.
  • 1.4 Provides fall prevention monitoring for patient safety.
  • 1.5 Provides bedpans and urinals at the request of the patient and in anticipation of the patient needs.
  • 1.6 Answers call lights within a timely manner and participates in hourly rounding.
  • 1.7 Constant observation for patients with altered mental status or patients requiring 1:1 observation. Alerts the nurse of potential concerns.
  • 1.8 Measures and reports data from nasal and abdominal tube drainage at the direction of the Registered Nurse and in accordance with hospital procedures.
  • 1.9 Pass diet trays and assists patients. Provides patients with nourishments in accordance with routines.
  • 1.10 Adheres to universal precautions as outlined in the departmental Infection Prevention Manual.
  • 1.11 Performs hand hygiene in accordance with Infection Prevention Manual.
  • 1.12 Assists with patient transfers, transportation and discharges. Assists patients with care of their personal belongings.
  • 1.13 Obtains and enters data in the electronic medical record: vital signs (including blood pressure), intake, output and weighs the patient in an accurate and timely manner.
  • 1.14 Assists with glucometer under the direction of the nurse.
  • 1.15 Assists with obtaining twelve (12) lead EKG as directed by the RN.
  • 1.16 Assists with postmortem care.
  • 1.17 Demonstrates safe technique for ambulation, range of motion (ROM), transfers and use of assistive devices when providing patient care.
  • 1.18 Assists patients with ambulation and exercises.
  • 1.19 Assists with ambulatory needs by demonstrating assistive walking devices (walkers, crutches, and canes).
  • 1.20 Identifies elimination need by observing for bladder distension, checking the patency and position of catheter tubing and drainage.
  • 1.21 Observes and notifies the Registered Nurse regarding the patency of nasal catheters, cannula, mask and reinforcing the use of the incentive spirometer.
  • 1.22 Assists with the procurement of specimens as directed by the nursing staff and in accordance with procedure.
  • 1.23 Functions independently with treatments, procedures and equipment within the parameters of the Nursing Assistant''s Skills and unit specific competencies
  • 2. Provides direct patient care as necessary following the accepted procedures and in accordance with the Medical Assistant skills. including but not limited to: vital signs, phlebotomy, assisting patients on and off stretchers and beds.

  • 7.3 Uses the staff elevators to transport patients.
  • 8. Presents a professional image.
  • 8.1 Wears name-badge in a highly visible location; adheres to department dress code; makes sure that appearance is appropriate for professional role.
  • 8.2 Practices good personal hygiene and grooming. Is neat and clean. Avoids colognes, perfumes and after-shave lotions.
  • 8.3 Communicates clearly and positively with coworkers and positively represents Greenwich Hospital in the workplace and our community.
  • 9. Answers the telephone professionally.
  • 9.1 Answers the phone promptly following department guidelines; introduces self and department, and as appropriate, offers assistance. Uses a pleasant tone of voice.
  • 9.2 Is a good listener; focuses on caller''s needs, clarifies as necessary; follows through on the caller''s requests.
  • 9.3 Uses the conference key to transfer calls; explains where the call is being transferred to and the extension for future use; gives information to the receiving department about the caller''s needs; brings caller on-line and introduces them.
  • 9.4 Uses communication technology appropriately, (beepers, voice and email, intercoms, overhead pages, etc.).
  • 10. Maintains a clean and safe environment.
  • 10.1 Keeps work area safe and clean; keeps hallways clear.
  • 10.2 Corrects unsafe conditions if possible, or, reports them if assistance is required.
  • 10.3 Understands the safety codes and knows how to respond effectively. Knows the location of the Department Safety Manual and is familiar with its contents.
  • 10.4 Uses equipment correctly and reports problems promptly.
  • 11. Anticipates what services and information people need and takes action to provide it.
  • 11.1 Watches for clues that patients or visitors require assistance. Asks, ''May I help you?'' Escorts them to their destination, if necessary. Encourages people to ask questions.
  • 11.2 Minimizes the anxiety caused by waiting by explaining the reason for the delay to the patient. Communicates with the patient frequently.
  • 11.3 Is sensitive to different cultural, emotional and spiritual needs of people.
  • 12. Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient''s status and interpret the appropriate information needed to identify each patient''s requirements relative to his or her age specific needs, and to provide the care needed as described in the unit''s/area''s/ department''s policies and procedures
    Other information:

    EDUCATION:

    A minimum of a high school diploma or GED; and CNA, PCT, or other educational equivalent, i.e. Medical Assistant, Phlebotomy Technician, Student Nurse who has completed one semester of clinical experience.

    EXPERIENCE:

    English speaking, basic mathematics and computer skills and knowledge of basic patient care.

    SPECIAL_SKILLS:

    Ability to read, write, basic computer skills and understand the English language; ability to perform basic mathematics; ability to carry out directions; must possess a mature, sympathetic and caring attitude with the ability to be sensitive to the varied needs of the GH patient population; must be flexible, cooperative, and able to meet the demands of the NA position using appropriate communication techniques; must pass the Greenwich Hospital Competency Program before providing patient care independently.

    PHYSICAL_DEMAND:

    May be exposure to infectious diseases. Moderate stress and risk of injury from patient care.
    Standing/walking for more than half of the day. Must be able to assist in pushing/pulling/lifting patients ranging from 5 to 300 pounds.
