A professional licensed Physical Therapist who is responsible for evaluation and treatment of patients, development and follow
through of treatment plans to address patient needs in accordance with hospital policies and APTA standards of practice. Provides
supervision to physical therapy assistants and support staff.
Performs a comprehensive evaluation as evidenced by the clinical documentation.
\u2022 Evaluation is comprehensive and appropriate to patient's age and diagnosis.
\u2022 Formulates goals that are time oriented, objective, and measureable.
\u2022 Develops a plan of care to promote optimum function and improvement that is consistent with identified problems.
Evaluations are clearly written or dictated and required information included (onset date, diagnosis, social history, appropriate
objective data, pain, assessment/ problem list, goals with time frames, education goals, patient/ family goals, barriers to
learning, treatment plan.)
Demonstrates competence in all physical therapy modalities and procedures in accordance with departmental policies.
\u2022 Demonstrates competence with use of equipment in department.
\u2022 Applies modalities in proper manner and properly adjusts and sets equipment prior to use by patients.
\u2022 Demonstrates awareness of precautions and contraindications for modalities.
\u2022 Monitors patient tolerance/ response and appropriately adjusts intervention as needed.
Documentation of evaluations, re-evaluations, and progress notes is professional, concise, accurate, and timely.
Is responsible to write legible, concise, relevant, timely, and inclusive documentation that communicates care provided, patient
status, and ongoing needs.
Documents periodic reassessments that include objective data, progress towards established goals (within time frames), and
ongoing/ revised treatment plan
Writes periodic physician progress reports, monthly recertification forms. Obtains physician certification for continued treatment
in timely manner.
\u2022 Writes or dictates a timely discharge note that includes discharge status and addresses goals.
Demonstrates leadership skills in supervision of physical therapy assistants and aides.
Appropriately delegates patient care interventions to PTA following APTA guidelines. Is aware of PTA schedules and regularly
shares patients with assistants.
\u2022 Provides instruction, follow up communication, and support as needed.
\u2022 Performs required supervisory observations with patient and PTA, as evidenced in the medical record.
\u2022 Demonstrates responsibility in supervision of supportive personnel (aides) in patient care.
Knows how to coordinate work. Works in an organized and efficient manner.
\u2022 Displays ability to treat more than one patient at a time as modalities and treatments allow.
\u2022 Responds to changes in the work schedule, demonstrates flexibility in view of interruption.
\u2022 Informs supervisor/ director of changes in work schedule, coverage needs, and patient issues.
\u2022 Keeps schedule current and neat.
