A registered professional nurse who utilizes the nursing process in providing nursing care and assumes the responsibility and accountability for prescribing, delegating and coordinating that care; assumes the responsibility for the direction of patient care through the resource nurse role; acts as a mentor for staff and students; provides programs for select staff.
Responsibilities: 1. Demonstrates skill in assessment of health care needs. 1.1 Demonstrates comprehensive skills in observing & collecting health care data through interviewing, observing, performing systems assessment & identifying patient support system on admission according to hospital policy. 1.2 Utilizes multiple data sources, such as other health care disciplines to identify obvious and obscure, physical, psycho social, educational and spiritual needs. 1.3 Identifies appropriate nursing diagnosis & expected outcomes based on ongoing assessment of patient health care needs. 1.4 Assesses patient health care needs reflecting changes in patient health status in an ongoing manner. 1.5 Demonstrates appropriate assessment of patient health care needs prior to discharge. 1.6 Demonstrates assessment of patient on transfer/discharge from unit. 2. Demonstrates skill in planning for patient health care needs. 2.1 Initiates clinical pathway utilizing information from initial assessment and guidelines for nursing care according to hospital policies. 2.2 Initiates patient care conferences, coordinating and collaborating with the patient, family and other members of the health care team. 2.3 Revises and individualizes clinical pathway to reflect change in patient''s condition. 2.4 Collaborates with other agencies involving patient, family, and/or significant others and health care team members in planning for patient education. 2.5 Involves patient, family, and members of health care team in discharge planning. 2.6 Involves peers and other health care team members in planning and revising the clinical pathway. 2.7 Effectively communicates with the Case Manager regarding discharge planning activities for assigned patients. 3. Demonstrates skills in implementing patient care in order to meet patient care needs. 3.1 Prioritizes patient care based on assessment. 3.2 Delegates tasks appropriate to members of the healthcare team & assists others in the delegation process. 3.3 Provides patient care based on nursing diagnosis & orders. 3.4 Demonstrates nursing skills appropriate to clinical area. 3.5 Demonstrates knowledge of tests and procedures & participates in patient education of same. 3.6 Independently develops and implements, evaluates & revises a teaching plan based on patient/family educational needs as evidenced by patient feedback. 3.7 Anticipates emergencies based on nursing assessment & takes immediate and appropriate action. 3.8 Facilitates informed decision making with the patient and family. 4. Demonstrates skill in evaluating care delivered. 4.1 Assesses patient responses to medical and nursing interventions based on expected outcomes. 4.2 Reviews and evaluates patient condition with physician and other health care professionals against expected outcomes. 4.3 Evaluates results of health care counseling and teaching as evidenced by feedback from patient, family, and/or significant others and appropriate documentation. 4.4 Reports pertinent and concise information to members of health care team. 4.5 Provides guidance and assistance to others in organizing & prioritizing patient care activities. 4.6 Considers alternatives for nursing care based on evaluation of patient responses and outcomes to established interventions. 4.7 Assumes role of Quality Management representative or actively participates in issue/problem resolution related to unit-based or hospital-wide QA/QI. 5. Documents outcomes appropriately, accurately and consistently. 5.1 Completes Admission Assessment Forms. 5.2 Maintains patient records, ie: transcription, CDS, pathways. 5.3 Accurately records patient data on flowsheets and MAR. 5.4 Performs discharge and transfer documentation completely. 5.5 Maintains unit-based statistical logs and information. 6. Performs decentralized laboratory testing, as credentialed and following established policies and procedures. 6.1 Appropriately collects specimens, performs test and interprets results. 6.2 Documents results and reports to physician as appropriate for patient care. 7. Demonstrates leadership skills and professional growth. 7.1 Identifies areas in performance needing improvement, sets specific realistic goals for self-development & sets target dates for completion through ongoing communication with supervisor. 7.2 Demonstrates and utilizes information in clinical field and shares information with peers. 7.3 Reviews and evaluates goals, as appropriate and discussed with supervisor. 7.4 Accepts constructive criticism & responds by implementing corrective action. 7.5 Effectively & appropriately articulates unique characteristics of situations. 7.6 Utilizes correct resources for clarification of policies & procedures. 7.7 Demonstrates knowledge & skills in acting as a resource person for other members of the health care team. 7.8 Demonstrates appropriate knowledge & skill for the resource nurse role. 7.9 Participates in the development or revision of clinical pathways specific to the unit. 7.10 Demonstrates skill in role of mentor for new staff in the development of clinical goals, completes competencies, and meets with mentee and NM on a regular basis. 7.11 Participates in the development, revision, and promotion of unit philosophy, goals & objectives. 7.12 Delegates effectively to other team members. 7.13 Acts as a role model for Professional Nurse I, CCPs and other members of the health care team. 7.14 Supports team building by consistently collaborating with all members of the team. 7.15 Demonstrates sensitivity to diverse cultural needs.
Graduation from an accredited School of Nursing. BSN or higher degree preferred. If non BSN, must show evidence of active enrollment in minimum of a BSN program.
Minimum of one year of Emergency Department RN experience required.
Licensed and a currently registered nurse in the State of Connecticut. Basic Cardiac Life Support (BCLS) training must be obtained prior to employment or within one (1) month of hire. ACLS and TNCC required.
Demonstrated evidenced of interpersonal skills. Successful completion of orientation.
All staff members must meet the criteria for their clinical level on an annual basis, as defined in the Professional Ladder. The individual nurse must submit written documentation to their manager, a checklist will be provided on the intranet under the Shared Governance site supporting maintenance of advanced clinical status to the Manager. The nurses supporting documentation must be submitted thirty days before annual evaluation is due or by the nurse managers discretion. The checklist will then be sent to the director for their signature and filed in your personnel folder. Failure to meet criteria by the evaluation due date will result in a demotion that will have an impact on pay rate. The employee may voluntarily withdraw their clinical ladder status and be demoted. This eval must be accompanied by documentation of all the activities performed during the eval year as listed below and on checklist. The following performance standards MUST be met annually to maintain Professional Nurse Status. Should these criteria stand unmet, the PN2 status is reduced to a PN1, as appropriate to the
standards that have been met. (Continued below in Accountability)
1. Active member of unit-based, divisional or hospital-wide committee, with evidence of participation and attendance of 75% of meetings within 6 calendar months of application. Provide a letter from the Committee chairperson summarizing participation. ** Use template provided.
2 . Participates in staff meeting by presenting above committee work. This should be reflected in a summary of staff meeting minutes. Provide a copy of staff meeting minutes.
3 . Attend a minimum of 4 in-services, Provide copies of in-service sign in sheets or copy of my transcript tab from health stream from those in-services you have attended. (Must be educational opportunities outside of health stream, skills days and policy reviews.)
4 . Provide documentation of one of the following:
--Earn 10 contact hours related to practice (category 1 CME may be used, however may not make up more than 50% of the total required).
--Earn 3 college credits toward a BSN OR post-graduate degree, OR bachelor''s in a field related to human sciences AND earn 6 contact hours related to your area of practice. Must submit proof of course completion. Provide copy of transcript and contact hour certificates
--Attain a national certification in the specialty area of practice, as defined by that specialty''s governing body, AND, earn 10 contact hours per year.
5. Provide a list of those students, interns or new RNs in whose orientation you have participated.
6. Function as a role model on home unit, coordinating unit activity in a collaborative, team-oriented approach across departments, units and disciplines.
7 . Maintain and update knowledge of clinical practice by reading professional nursing literature as evidenced by a reference list of at least five articles. Articles should be taken from literature less than five years.
8 . Plan and present one evidence-based in-service to peers complete with documentation of objectives, brief content description, teaching methods, rationale for choosing the topic, attendance sheet showing participants and reference list . Articles should be taken from literature less than five years old.* This requires your own original work. For in-services done in conjunction with staff education implementing the Train the Trainer model of education, original reference list from staff education is acceptable. ** Use template provided.
9 . Describe your role in the hospital''s strategic plan. This should reflect how you have supported and helped the organization achieve the four pillars, of the strategic plan within your role on your unit or within the organization. No more than two pages in length.
