Job Description

A cardiac rehabilitation nurse utilizes the nursing process to offer a multifaceted and multidisciplinary approach to overall cardiovascular risk reduction, foster healthy behaviors, reduce disability, and promote an active lifestyle for patients with cardiovascular disease. The nurse serves as a liaison between patient and physician to facilitate expeditious and effective medical care. Participates in running Project Heart Smart when offered and in the Heart Beats Volunteer group as needed. Essential duties and responsibilities include the following. Other duties may be necessary and assigned.
  • 1. Demonstrates skill in assessment of health care needs.
  • 1.1. Demonstrates comprehensive skills in observing and collecting health care data through chart review. Obtains appropriate clinical information for thorough review.
  • 1.2. During initial nursing assessment, identifies historical, recurring and present signs and symptoms of impaired oxygenation from cardiopulmonary disease and documents in medical record according to hospital policy.
  • 1.3. Utilizes multiple data sources, such as other health care disciplines to identify obvious and obscure physical, psycho social, educational, spirtual, cultural and familial needs of each individual patient.
  • 1.4. Identifies appropriate nursing diagnosis and expected outcomes based on ongoing assessment of patient health care needs.
  • 1.5. Documents and communicates assessment findings to cardiac rehabilitation team members, medical program director and family or other health care providers as deemed necessary in a timely and efficacious manner.
  • 1.6. Assesses current cardiopulmonary status, goal achievement in program, psychosocial status, lifestyle and other concerns in the development of a short term and long term discharge plan.
  • 2. Demonstrates skill in planning for patient health care needs
  • 2.1. Plans an appropriate exercise routine and progression pattern including warm-up and cool down exercises according to individual patient needs an program protocol.
  • 2.2. Develops education plan for patient based on initial and ongoing assessment utilizing individual treatment plan template.
  • 2.3. Performs ongoing patient assessment during exercise as evidenced by documentation in patient record.
  • 2.4. Documents patient's progress and communicates observations to physician as evidenced by feedback from physicians.
  • 2.5. Initiates/performs emergency procedures according to hospital policy as required as evidenced by documentation in patient record.
  • 3. Demonstrates skills in implementing patient care plan in order to meet patient care needs.
  • 3.1. Prioritizes patient care based on nursing care assessment , individual treatment plan, team meetings, and ongoing assessment of patient needs.
  • 3.2. Delegates tasks appropriate to members of the health care team and assists others in the delegation process.
  • 3.3. Demonstrates nursing skills appropriate to clinical area and follows all policies and procedures.
  • 3.4. Demonstrates cardio vascular exercises, weight training exercises and stretching exercises while teaching the importance and necessity of and relationship to cardiovascular health.
  • 4. Demonstrates skill in evaluating care delivered
  • 4.1. Maintains files and statistics according to protocol as observed by supervisor.
  • 4.2. Records information required for accurate billing according to hospital policy as evidenced by random supervisory review of billing records.
  • 5. Continually strives to improve the safety and quality of patient care.
  • 5.1. Participates in departmental quality activities and provides input and suggestions to improve patient safety.
  • 5.2. Continuously evaluates working condition and safety of equipment and cardiac rehabilitation space and takes appropriate action to ensure safe conditions.
  • 5.3. Promotes patient safety on exercise equiptment by teaching/demonstrating proper use of exercise equiptment andsupervising and coaching patients on proper usage of aerobic equiptment and weights
  • 5.4. Individualizes patient safety education to suit patient needs.
  • 6. Demonstrates leadership skills and professional growth.
  • 6.1. Maintains files, documents appropriately, and records statistics according to protocol as observed by supervisor.
  • 6.2. Routinely evaluates working condition and safety of equiptment and takes appropriate action.
  • 6.3. Participates in departmental quality assurance activities as evidenced by statistical outcomes.
  • 6.4. Maintains current knowledge of all new recommendations for secondary prevention of cardiovascular disease by keeping current with availiable resources.
  • 6.5. Keeps current and up to date with computer skills, exercise recommendations, time management skills, group dynamic skills and group presentation skills.
  • 6.6. Continually seeks new opportunities to expand knowledge of secondary cardiac prevention, communication and leadership skills to improve skill set.
  • 6.7. Attends all hospital inservices that are mandated.
    Other information:

    Registered Nurse in the state of Connecticut. Bachelors degree preferred (enrollment required). Acute care experience. Telemetry trained. Exercise training, group dynamic skills or experience related to primary and secondary cardiac prevention.
    Cardiology, physical rehabilitation, community health, teaching, exercise training or public health training .
    Licensure as a registered nurse in the state of Connecticut, Telemetry trained, Advanced Cardiac Life Support, Certification by American College of Sports Medicine in exercise therapy.
    Advanced knowledge of cardiovascular disease, telemetry trained, exercise physiology, cardiac risk factor modification, counseling techniques, nutrition, group dynamic and cardiovascular health education principles.
    Physical assessment skills, triage capabilities, psychological assessment skills, group dynamic skills, public speaking skills, physical demonstrations of strength training, weight lifting and aerobic exercises, personal role modeling of cardiovascular health behaviors, communication skills.
    While performing the duties of this job, the employee is regularly required to stand, sit, walk, talk and hear. The employee is also required to demonstrate and model aerobic, strength training and flexibility exercises to classes. The noise level in the work environment is moderate. the employee should be a passionate advocate of a Heart Heathy Lifestyle.
