Summary:
A Licensed Practical Nurse who gives nursing care to patients and performs additional functions under the supervision of the physician/provider. Conveys a positive image of the practice.
Responsibilities:
  • 1. Performs patient care responsibilities as assigned
  • 1.1 Identifies self to patient/family and irents patient/family to their roles of each member of the care delivery team.
  • 1.2 Accompanies patients to exam/procedure room.
  • 1.3 Prepares patients for visit. Typically includes, vitals, height, weight, blood pressure, EKG, specimen collection (where applicable), phlebotomy (where applicable), INR (where applicable), pulse ox, spirometry, etc. Documents history, family social, problem list, medications, in Epic.
  • 1.4 Administers medications via injections or orally.
  • 1.5 Thoroughly explains ordered tests to patients.
  • 1.6 Schedules medical testing at appropriate facilities and explains instructions to the patient.
  • 1.7 Educates patients on prescriptions.
  • 1.8 Performs screening per physician/provider guidelines.
  • 1.9 Assists physicians/providers with various procedures.
  • 1.10 Obtain pre-authorizations when necessary.
  • 1.11 Relays instructions to patients/family members.
  • 1.12 Provides appropriate resource materials and referrals to patient/family as instructed by physician/provider.
  • 1.13 Maintains patient records effectively.
  • 1.14 Efficiently manages and routinely checks Epic in-basket.
  • 1.15 Answers calls and return calls (in a timely manner) and provides pertinent information.
  • 2. Performs administrative responsibilities as assigned.
  • 2.1 Sends/receives patient medical records and abstracts new patient records into Epic.
  • 2.2 Obtains lab/radiology reports, hospital notes, referral information as requested.
  • 2.3 Completes forms/requisitions as needed.
  • 2.4 Cross trained on front desk duties including but not limited to, greeting patients/visitors, answering calls, scheduling appointments, maintaining/scanning patient medical records
  • 3. Performs environmental responsibilities as assigned.
  • 3.1 Sets up instruments and equipment according to department protocol.
  • 3.2 Applies principles of infection control.
  • 3.3 Cleans exam/procedures rooms, instruments and equipment between patient visits to maintain infection control following established department policy.
  • 3.4 Cleans sterilizer (when applicable) according to scheduled maintenance program and maintains appropriate documentation.
  • 3.5 Orders, sorts and stores supplies following established department policy.
  • 3.6 Restocks exam/procedure rooms as required.
  • 4. Performs other related organizational duties as assigned.
  • 4.1 Maintains patient autonomy, confidentiality and dignity.
  • 4.2 Ensures providers submit timely and accurate billing.
  • 4.3 Maintains knowledge of practice policy, HIPPA, current coding, OSHA regulation and CLIA regulations (where applicable).
  • 4.4 Provides prompt attention to urgent and non-urgent patient issues as evidenced by observation and feedback from staff.
  • 4.5 Quickly shares problems related to patients and/or staff with Supervisor.
  • 4.6 Work with Care Coordinator for Patient Centered Medical Home initiatives
    Other information:

    EDUCATION:

    High School Diploma and LPN Diploma Required.

    EXPERIENCE:

    2 years of medical practice experience preferred.

    LICENSURE:

    Current licensure from the State of Connecticut and/or New York (when necessary).

    SPECIAL_SKILLS:

    Ability to assist physicians in minor procedures (where applicable). Excellent, organizational, communication, customer service and teamwork skills required. Ability to multi-task is essential. EMR experience preferred and working knowledge of Microsoft office applications required. CPR certification preferred. Bilingual preferred.
    ACCOUNTABILITY:

    Must complete work in a timely fashion.
    COMPLEXITY:

    PHYSICAL_DEMAND:

    MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids.
    Tdap and influenza vaccination are strongly encouraged.
