Job Description

Flex Tier RN Program for the following specialty areas:
\u2022 Med/Surg General Nursing
\u2022 ICU/Stepdown

This position requires a minimum schedule of 32 hours a month after the orientation period has concluded.

Work requirements also include 12 shifts per year of either off-shift or weekends, as well as minimum two holidays per year.

Tier levels are based on the number of specialty areas in which candidates can demonstrate competency.

Tier 1 - $40/hour plus applicable shift differentials - evaluated as competent in one specialty area
Tier 2 - $43/hour plus differentials - two specialty areas

Ideal Candidates will demonstrate the following skills/experience:

\u2022 Graduation from an accredited School of Nursing (BSN preferred)
\u2022 Active Registered Nurse licensure in the State of Connecticut
\u2022 Current BCLS certification or must obtain within one month of hire
\u2022 Minimum - two years of recent acute care experience
\u2022 ICU specialty - one year of direct ICU experience

Summary:
As a core member of the Care delivery team, the Clinical Nurse practices as the patients nurse and collaborates with the team to facilitate the coordination of patient care at the unit level. Through utilization of the nursing process, the clinical nurse diagnoses and treats patient and family responses to actual or potential health problems. The Clinical Nurse is accountable to the patient, family, peers, and Manager.
Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based on the 4 principles of PFCC: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence by creating a great "First Impression" by demonstrating exemplary customer service skills for all customer groups including patients, families/friends, physicians, staff and support department personnel.

Responsibilities:
  • 1. Nursing Process
  • 1.1. The Clinical Nurse I (CN I) utilizes the nursing process of assessment, planning, intervention and
    evaluation while caring for our patients maintaining confidentiality and dignity. The Clinical Nurse
    provides safe, effective evidenced based care tailored to the individual needs of the patient and family.
    As a core member of the care delivery team, the Clinical Nurse partners with the team to facilitate the
    coordination of patient care at the unit level utilizing the principles of patient/family centered care. The
    clinical nurse uses a 5 step process to deliver high quality comprehensive and individualized care
    incorporating.

    1. Patient assessment
    2. Problem identification
    3. Establishment of desired outcomes
    4. Implementation of interventions
    5. Evaluation of care with necessary revisions.

  • 3.1. Collaborates with members of the multidisciplinary team to support the work of the team, unit
    activities, and the accomplishment of positive patient outcomes.

  • 4.1. Delegates responsibilities to appropriate members of the care delivery team based upon an
    understanding of others' roles, scope of practice, knowledge, and capabilities to ensure optimal
    utilization of personnel.

  • 4.3. Maintains a collaborative work relationship with staff members through respect and recognition of
    cultural and human diversity.

    EDUCATION: Graduation from an accredited School of Nursing, BSN preferred., Licensed and a currently registered nurse in the State of Connecticut., All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire. EXPERIENCE: Minimum 2 years RN experience for Med/Surg Flex Tier, minimum 1 year ICU experience for ICU Flex Tier Clinical Nurse is the basic level for new graduates and all registered nurses ., All RNs are expected to advance to their optimal level of functioning through promotion. . LICENSURE: CT RN licensure or eligible SPECIAL_SKILLS: Excellent communications, interpersonal, coordinative, and organizational skills
    ACCOUNTABILITY: Provides nursing care to an individual or group of patients; recognizes changes in patients condition and reports such changes to Physician and/or Patient Services Manager., Utilizes and manages material resources in a cost effective manner.
    COMPLEXITY: Plans, develops and implements patient care plans; evaluates the effectiveness of existing patient care; evaluates the effectiveness of existing patient care plan and modifies plan of care to meet patient needs, Please see RCAP CMP for further details.
