Summary:
As members of a High Reliability Organization (HRO), every employee should practice CHAMP behavior and strive to make patient safety a top priority. Pharmacy Operation Specialist supports the vision, mission, and goals of YNHH and the Department of Pharmacy while demonstrating proficiency in overseeing the day to day operation, ensuring safe, efficacious and timely drug therapy. Act as a liaison between Pharmacy and other YNHH Departments on issues of medication distribution, medication safety and clinical initiatives. Pharmacy Operation Specialist must have met all general requirements of Clinical Pharmacist and Clinical Pharmacist 2 positions in addition to specific pharmacy practice, drug information, educational/scholarly, and leadership requirements.

Responsibilities:
  • 1. Assist pharmacy management team in coordinating the day to day activities ensuring an efficient and safe workflow through the enforcement of both departmentaland organizational policies and procedures.
  • 2. Demonstrate a thorough understanding of the medication distribution model and its supporting technologies and pharmacy systems. Oversee the operational effectiveness of such technologies and systems and escalate issues/concerns via the appropriate channels in a timely manner.
  • 3. Regulatory and Safety Quality Compliance
  • 3.1. Performs regulatory/compliance audits in the Pharmacy areas where medications are stored, prepared or administered to identify practices that deviate from existing policies or standards. Assists in the implementation of process improvement initiatives and action plans to address practice gaps and foster a culture of safety.
  • 3.2. Effectively demonstrates the principles and conduct consistent with a High Reliability Organization.
  • 4. Assist in the oversight of controlled substances as it pertains to procurement, storage and distribution throughout the organization. Contribute to the ongoing monitoring of medication utilization ensuring the appropriate disposition of all controlled substances
  • 5. Participates in the orientation and training of professional and technical staff
  • 6. Drug Therapy Management
  • 6.1. In a timely fashion, reviews and verifies all medication orders to ensure safe, accurate, cost effective drug use
  • 6.2. Evaluates patient specific drug therapy management plans and therapy endpoints and suggests changes and improvements.
  • 6.3. Understands formulary process. Identifies and recommends alternative drugs for requests for non-formulary drugs.
  • 6.4. Performs pharmacist responsibilities as outlined by pharmacist protocols (e.g. IV to PO protocol, renal dose adjustment protocol, aminoglycoside and vancomycin monitoring protocols, and others)
  • 6.5. Consistently documents pharmacist interventions and follows through on process resolution.
  • 6.6. Consistently attends medical/interdisciplinary rounds.
  • 7. Interpretation of Clinical & Pharmacokinetic Lab Data
  • 7.1. Demonstrates knowledge of pharmacokinetic and pharmacodynamic principles and applies to recommend changes in medication regimens.
  • 7.2. Performs pharmacokinetics calculations. Recommends drug therapy changes to prescribers, and prospectively initiates appropriate dosage regimens.
  • 7.3. Identifies targeted serum drug levels/lab values requiring pharmacist intervention
  • 7.4. Evaluates the efficacy of drug therapy and anticipates side effects, toxicity or adverse effects.
  • 7.5. Evaluates the effect of organ systems function on drug effect and recommends dose adjustments accordingly.
  • 8. Medication-related issue identification and resolution
  • 8.1. Consistently documents Medication Use Variances
  • 8.2. Identifies medication-related issues, communicates with proposed solutions to pharmacy leadership, and implements solutions.
  • 8.3. Resolves medication-related issues through medication use guidelines, CPOE screen changes, and operational process improvement initiatives.
  • 9. Drug Information
  • 9.1. Provides drug information to physicians, nurses, and other health care professionals.
  • 9.2. Possesses comprehensive knowledge of drug information resources and applies information to specific patient cases.
  • 9.3. Identifies the need and creates new IV guidelines; updates existing IV guidelines based on new data/practice changes.
  • 10. Communication and Service Requirements

  • 10.3. Able to effectively communicate information to patients and their families.

  • 11. Education/Scholarly Activities
  • 11.1. Performs patient counseling
  • 11.2. Serves as primary preceptor for pharmacy students
  • 11.3. Serves as co-preceptor or primary preceptor for pharmacy resident research projects
  • 11.4. Trains and mentors new pharmacists
  • 11.5. Consistently attends intra-departmental pharmacy conferences
  • 11.6. Provides at least two in-services to pharmacy staff annually
  • 12. Leadership
  • 12.1. Consistently attends department meetings
  • 12.2. Actively participates on an interdisciplinary hospital committee.
  • 12.3. Actively participates on a Pharmacy Department committee.
  • 12.4. Participates in clinical, process improvement, cost savings, or safety projects
  • 12.5. Demonstrates community service involvement (as part of pharmacy departmentfYNHH activity or for a health-related cause).
  • 12.6. Active member of a local or national pharmacy society (leadership role preferred)

    Other information:

    EDUCATION:

    Graduation from an accredited College of Pharmacy with a B.S., M.S., or Pharm.D. degree.

    EXPERIENCE:

    Completion of an ASHP accredited PGY1 pharmacy residency and PGY2 pharmacy residency OR equivalent experience is preferred but not required. Each residency is equivalent to 2 years of post-graduate experience.

    LICENSURE:

    Active Connecticut Pharmacist License. Board of Pharmacy Specialties Certification is preferred, but not required.

    SPECIAL_SKILLS:

    This individual should demonstrate in-depth knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and formulary management. Experience in change management, and managing people is highly desirable. Experience in drug utilization review and data analysis is highly desirable . Excellent written and verbal presentation skills are required.
    ACCOUNTABILITY:

    The individual in this position is held accountable to maintaining the highest of professional standards and promoting rational and cost-effective therapy to positively influence patient outcomes. Successful or outstanding performance on all annual review goals and behaviors is required. Must complete all hospital and department competencies and requirements for employment and maintain annual continuing education credits as mandated by the State of Connecticut Department of Consumer Protection.
    COMPLEXITY:

    The position requires leadership qualities and the ability to effectively communicate, both written and verbal. Must apply new ideas, knowledge and skill gained from experience and education to current situations; and should possess a superior knowledge of current standards of pharmacy practice. Must utilize sound clinical and operational judgment in making therapeutic and technical decisions.
