The Pediatric Clinical Nurse Practitioner provides comprehensive care to patients and their families within a specific pediatric specialty (examples include: Diabetes, Endocrinology,Genetics, Neurology, Orthopedics) under the supervision of the Attending Physician of the assigned specialty, and as directed by established standards of care. As a member of the health care team, the Nurse Practitioner provides direct care, and focuses on education to patients and families regarding prevention and/or management of their health status within YNHCH Pediatric Specialty. The Pediatric Nurse Practitioner works collaboratively with all members of the specialty service and multidisciplinary healthcare team to provide safe,high-quality, evidenced-based care. Some specialtiies many require coordination and/or support in clinical research trials.
This position will function in the inpatient setting for diabetes education and care of new/follow up patients. In addition the role functions as an independent provider seeing outpatient specialty visits at New Haven and Trumbull Pediatric Specialty Centers.
Responsibilities: 1. Patient Care: 1.1. Assumes primary patient care responsibilities for patients in the YNHCH Pediatric Specialty Service,specifically relating to their specialty and patient populations. 1.2. Identifies health care needs and assesses health status of patient/family during outpatient clinic visits. Includes performing a history (medical, family, and social) and physical examination for specific patient population, including new or return visits. 1.3. Identifies medical and psychologicial patient problems and develops and implements a healthcare plan in collaboration with Attending Physician and multidicsiplinary team as needed. 1.4. Ensures complete documentation, includes all elements of regulatory and third party carrier requirements as evidenced in the medical record. Includes but not limited to: History and Physical exam, Problem list, Allergies, Medication reconciliation, Plan of care. Documentation includes patient education and interval contact with patients through telephone. 1.5. Discusses patient care needs with Attending Physician and members of the multidisciplinary healthcare team (as needed) during regular scheduled specialty, interim care needs, or during mulitdisciplinary meetings. 1.6. If appiclable to specialty, provides on-call service as apropriate to meet the needs of the specialty. 1.7. Utilizes age-specific developmental competencies in the development of patient care plans 1.8. Develps the plan of care (seeks validations with the attending physician or other appropriate specialists) utilizing pertinent health issues based on physical status , laboratory results, radiology results, patient responses to treatment an/or medication management. 1.9. Independently performs (or assists) with procedures as appropriate for specialty. Develops competency and is credentialed for procedures as per regulatory requirments and YNHH standards of practice. 1.10. Independently, or in collaboration with healthcare team members, orders, schedules, interprets, and dcouments laboratory or diagnostic tests and procedures, 1.11. Ensures follow up with patient/family regarding laboratory or diagnostic test results, including communication, and documentation of source for delivering results. In addition, counsels patient/family on plan of care/next steps. 1.12. In conjunction with the attending physician, evaluates and treats acute, urgent patient care needs and arranges for admissions to healthcare facilities as needed. 1.13. Initiates, coordinates, and manages home care needs for patients and families. 2. Research/ Co-investigator/coordinator for specialty Clinical Trials.
ONLY WHERE APPLICABLE: Research activities, in conjunction with Clinical care must be reviewied and approved by Specialty Section Chief, Dept of Pediatric and Surgery Administratiive leadership, and the Director of the Pediatric Specialty Centers to detrmine appropriate allocation of Clinical Research responsibilite. The extent of involvement and timeframe will be determined collaboratively with the APRN and the leadership for each Research project 2.1. Prepares and submits protocol, consents, and re-approvals as needed to HIC 2.2. Schedules and performs all patient visits in collaboration with studgy site staff. 2.3. Maintains all research records in accordance with HR and sponsor guidelines. 2.4. Assures responsibility for processing and shipping of biological samples (if applicable) 2.5. Communicates study results to patients and their health care providers. 2.6. Transmits data to study/sponsor. 2.7. Facilitates obtaining of consents for patient participation in studies, including administering of any intake questionaires, recording data 2.8. The practitioner may participate, collaborate or initiate research activities with the Yale Medical Group Specialties and Yale New Haven Hospital 2.9. Applies evidence and research -based data to clinical practice in the care of patients and families in the specialty area 3. Patient/Family Centered Care 3.1. Collaborates with other health care team members, referring physician, and coordinates appropriate patient/family education and follow-up care. 3.2. Embraces and supports family centered care with the patient and family as the center of all care management processes. Includes and engages the family in care plans and decision making process 3.3. In collaboration with the health care team, provides continuity of care to patients needs coordinating care in the inpatient and outpatient setting, accross disciplines, to meet patient and family needs. Includes but not limited to:
a. Collaborating wtih other services involved in the child's care i.e. primary and/or specialty providers, school nurses, nursing/DME agencies, insurance companies, inpatient and outpatient nurse coordinators
b. Completes necessary forms for schools, insurance companies, letters, as needed to support patient care needs
c. Initiates scheduling of referrals to other specialty services, ancillary support or testing needs and follow up appointments. 4. Education 4.1. Identifies learning needs of team members and families. 4.2. Provides education/support to patients, families, clinical staff on the inpatient or outpatient setting, homecare, and community (school nurses) 4.3. Participates in continuing education/preceptorship of health care team and affiliated medical and nursing students. 4.4. Identifies appropriate patient education materials for patient/family, including written, electronic, resosurce support groups, and development of educational materials as neeeded. 4.5. Monitors patient outcomes across the health continuum, tracking patient/family responses to prevention, management and treatment . 5. Professional Development: 5.1. Participates in at least one (1) professional organization. 5.2. Attends national and local conferences, and internal in-house conferences each year. 5.3. Maintains current nationally recognized certificates. 5.4. Maintains competent in YNHH and YMG competencies, standard policies, including compliance with regulatory requirements relating to patient care and environment, 5.5. Ensures compliance with provider credentialing and ongoing renewals
MSN - Graduate of approved Family/Nurse Practitioner Program. Preferrerably a Pediatric Nurse practioner program with concentration in specialty area. Current Connecticut APRN license Certification as FNP, or PNP. Specialty Certification preferred.
Two (2) years as a NP, PNP Preferred, or similar role in specialty area.
Excellent basic medical knowledge with clinical skills and judgement. Possess analytical skills to evaluate care needs, ability to gather relevant clinical data. Demonstrated independent decision making and the ability to initiate with minimal supervision. Must be able to perform thorough physical examinations. possess excellent interpersonal skills: Counselilng, communication, and leadership abilities. Must exhibit good judgement and tact dealing with parents, families, and allied health professionals. Computer skills essential
Must be able to understand basic principles of the specialty and ability to develop skill for assessments, as well as managment of chronic, acute, or chronic disease for patient and family, including arranging for and interpreting test results
