The primary function of the Advanced Practice Nurse will focus on the health and wellness of the Adolescent through Adulthood using a patient and family centered care model. Provides services to adult patients from early adult to the elderly for medical management of routine health maintenance, screening and health prevention, acute/chronic issues, education, and provision of psychosocial support. Collaborates with advocacy groups & local agencies. Community activities are required.
Responsibilities: 1. Clinical Practice: 1.1. Assumes primary patient care responsibility for patients in a specific program under the supervision of
the Attendings. 1.2. Identifies health care needs and assesses health status of patient/family during inpatient hospitalization
and subsequent outpatient care follow-up if appropriate. Monitors patients across the health care
continuum and develops strategies to avoid hospitalization and/or decrease length of stay. 1.3. Prescreens patients, obtains comprehensive health complete histories, and performs complete physical exams during treatment hospitalization and in visits. 1.4. Identifies and collaborates with the specialty interdisciplinary team in the design of patient treatment
plans. 1.5. Communicates patient status information, and documents progress in the Medical Record. 1.6. Orders appropriate tests and therapy; interprets results and outcomes; discusses appropriate interventions with the physicians. 1.7. In collaboration with the Health care team, provides continuity of care to patients in the outpatient settings to achieve optimal patient outcomes and patient satisfaction. 2. Education: 2.1. Identifies learning needs of team members, patients and families. Provides bedside teaching and supervision of procedures. 2.2. Acts as a clinical resource for residents and fellows, and as a role model/preceptor for nurses and advanced practice nursing students. Maximizes opportunities in the clinical setting for informal
education of staff, patients, and families. 2.3. Lectures at conferences and seminars for health care professionals. 3. Research: 3.1. May support the research mission through collaboration with investigators and protocol personnel. 3.2. May promote research utilization through the dissemination of existing and new research findings in the clinical practice setting. 3.3. May promote specialty clinical trials, both in-house and through cooperative groups.
Master's degree in nursing with Advanced Practice Registered Nurse as a Family Nurse Practitioner or Adult focus.
Three (3) to five (5) years experience in Adult or Family Practice. Previous experience in an Advanced Practice role required.
Licensed and a currently registered nurse in the State of Connecticut. RN/APRN Licensure.
Excellent interpersonal and communication skills. Knowledge of research methods, computer skills required.
Accountable to patients, families, peers, colleagues for clinical practice that upholds the mission, philosophy, objectives and policies of Patient Care Services and Yale New Haven Hospital. Reports to the Practice Administrator and the Medical Director for clinical practice.
Assesses and intervenes with patients, families, systems and community agencies. Collaborates with/assists staff with care of patients and families. Has clinical leadership responsibilities for quality patient outcomes and for innovation for approaches to clinical practice and health care delivery systems. Must be able to respond to unanticipated events effectively and be flexible, exercising superior problem solving skills and contingency management.
Possesses physical and mental heath necessary to care for patients and families. Possesses a full range of body motion, including lifting and handling of adult patients.
