Summary:
Performs selected patient care services and assists other care providers under the direction of the R.N.. Documents care on patient flow sheets. Essential duties and responsibilities include the following. Other duties may be assigned.

Responsibilities:
  • 1. Demonstrates skill in observing and communicating patient needs.
  • 1.1. Identifies changes in patient's condition and communicates significant observations to the nurse in a timely fashion asIndicated by report to the nurse.
  • 1.2. Notes deviations in established hospital routine affecting patients as evidenced by report to nurse.
  • 1.3. Documents accurately and completely on Patient Care Flow Sheet based on periodic supervisory review of the flow sheets.
  • 2. Demonstrates skill in providing direct patient care.
  • 2.1. Accepts report on assigned patients and demonstrates understanding as evidenced by her/his communication.
  • 2.2. Follows established procedure in providing routine patient care as observed by supervisor.
  • 2.3. Follows established procedure in performing assigned functions as observed by supervisor.
  • 2.4. Assists nurse in designated activities as observed by supervisor.
  • 2.5. Completes patient care in a timely manner as observed by supervisor.
  • 2.6. Assures assignment area is stocked with supplies and linen including specialty carts.
  • 3. Performs decentralized laboratory testing as credentialed.
  • 3.1. Appropriately collects specimen and performs test following established policy and procedure 100% of the time as observed by supervisor.
  • 3.2. Reports results to RN following established policy and procedures as observed by supervisor.
  • 4. Completes non-therapeutic functions as assigned by NM/designee.
  • 4.1. Follows correct procedure in performing non-therapeutic functions as observed by supervisor.
  • 4.2. Maintains a safe and clean environment in patient rooms and nursing work areas as observed by supervisor.
  • 4.3. Obtains equipment and supplies as observed by supervisor.
  • 4.4. Documents vital signs in patient record as requested based on a random review of charts by supervisor.
  • 4.5. Transports patient/specimens when necessary following established hospital policies and procedures to facilitate patient care as observed by supervisor.
  • 4.6. Assures work area is stocked with necessary supplies/linen including specialty carts.
  • 4.7. Remove medication(s) from the pneumatic tube system sent from the Pharmacy or Hospital Supply to the unit/patient care area.
  • 4.8. Immediately deliver the medication(s) from the pneumatic tube system to the appropriate Registered Nurse, Physician, Advanced Practice Registered Nurse, Physician Assistant or other appropriate licensed practitioner.
  • 4.9. As directed by a Registered Nurse, Physician, Nurse Practitioner, Physician Assistant or other appropriate licensed practitioner, remove IV solutions from the patient care area's clean utility room and immediately deliver to the appropriate licensed practitioner.
  • 4.10. As directed by a Registered Nurse, Physician, Nurse Practitioner, Physician Assistant or other appropriate licensed practitioner, stock IV solutions in the patient care area's clean utility room.
  • 5. Demonstrates accountability and responsibility in daily performance of role.
  • 5.1. Relocates and rotates to meet patient care needs as observed by supervisor.
  • 5.2. Completes patient care in a timely manner as observed by supervisor
  • 5.3. Completes direct patient assignment in a timely manner as observed by supervisor.
  • 5.4. Participates in a minimum of 8 unit staff meetings each year as observed by supervisor.
  • 5.5. Assumes responsibility for delegated activities as observed by supervisor.
  • 5.6. Completes mutually agreed upon goals in a designated timeframe as reviewed by supervisor.
  • 5.7. Maintains skills applicable to the unit as observed by supervisor.
  • 5.8. Records attendance at educational programs on appropriate form as observed by supervisor.
  • 5.9. Accepts constructive criticism and responds by implementing corrective action as observed by supervisor.
  • 6. Completes skills checklist.

    EDUCATION:

    High school diploma or equivalent. Certified Nursing Assistant/EMT/Certified Medical Response Technician/Certified Clinical Medical Asst./Nursing Student, LPN.

    While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. They are also required to stand and walk. Must be able to assist with pushing/pulling/lifting patients up to 75 pounds. If the lifting required exceeds 75 pounds and the patient is not ambulatory, assistance form other staff members must be obtained. Specific vision abilities include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. The noise level in the work environment is usually moderate.
