The Clinical Nurse Specialist Skin Health provides direct and indirect care to patients with wounds, ostomies, and incontinence. This position provides leadership by serving as a clinical resource and consultant for staff, physicians, and members of the interdisciplinary care team. Scope includes Clinical Consultation/Expert, Education, Leadership, Evidence-based practice, and Research. Responsible for bedside clinical practice support 24/7.
Responsibilities: 1. CLINICAL PRACTICE 1.1. Provides direct and indirect care to patients with diabetes and their families throughout the continuum of care. 1.2. Utilizes specialized knowledge base and advanced practice skills to provide, guide, direct and evaluate diabetes nursing care. 1.3. Integrates new technology, current literature and possesses distinct experiential knowledge to enhance diabetes nursing practice. 1.4. Monitors the progress of patients/families with diabetes needs towards goal achievement with staff, facilitating changes in care as necessary. 1.5. Facilitates continuity of care for diabetes patients both within and across settings. 1.6. Utilizes current specialty standards to solve complex clinical problems in the care of patients with diabetes needs. 1.7. Identifies educational needs of diabetes patient/families with diabetes and collaborates with the primary nurse and other health care professionals to meet these needs. 2. Leadership: 2.1. Provides staff functions for nursing administration. 2.2. Provides leadership to develop, implement and evaluate clinical standards, policies and 2.3. procedures related to diabetes nursing practice. 2.4. Participates in quality improvement activities related to diabetes nursing. 2.5. Collaborates with administrators to identify and correct system impediments to efficient diabetes 2.6. nursing practice and optimal patient care outcomes. 2.7. Participates with administrators and patient services managers to develop and implement 2.8. institutional, divisional, departmental or unit goals. 2.9. Participates in the development of strategic long range plans for the care of patients with diabetes 2.10. Participates in financial planning for diabetes care. 2.11. Provides consultation and assistance to staff in developing and implementing best practice in the identification and management of patients with potential or actual alteration in skin integrity 2.12. Provides guidance to staff in implementation of protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces 2.13. Maintains records and data related to work structure, process, and outcome statistics and summarizes and submits Analyzes inventory and recommends appropriate additions and deletions to ensure quality and cost-effectiveness of products used for skin and wound care 2.14. Promotes the organization to all customers by integrating and communicating Yale New Havens mission and values, acting as a loyal, supportive, and informed spokesperson for the Hospital. Identifies and communicates opportunities for collaboration that result in optimal patient care delivery. 2.15. Supports Patient Services leadership in the facilitation of inter- and intra-departmental problem resolution related to practice. Promotes, advises, and influences system level policy and procedure that will improve the delivery of patient care. 2.17. Provides appropriate education to patients, caregivers, and staff regarding skin care and wound management 3.2. staff to maintain current knowledge and competence in the areas of skin and wound care through orientation and regularly scheduled inservice education programs 3.3. Attends continuing education programs related to wound management 4. Research: 4.1. Assists staff to maintain current state of the art practice by reviewing and revising policies and procedures to be consistent with national guidelines and other sources of evidence-based literature 4.2. Conducts prevalence and incidence studies 4.3. Conducts product evaluations or contributes to research studies related to skin and wound care when indicated, and submits reports and recommendations based on results 5. Customer Satisfaction: 5.1. Support unit leadership in ensuring that customer service standards reflecting excellence consistent with Hospital policy are met. 5.2. Evaluates new therapeutic/preventative interventions that target patient needs not met by current care delivery strategies. 6.4. Based on direct involvement in clinical practice and patient care, determines whether variation or less then optimal practice patterns exist and facilitates the introduction of best practices to improve outcomes.
11.2Provides clinical consultation regarding patient care practices and advocates for changes in patient care processes and systems to facilitate optimal health care delivery. Formulates recommendations to Patient Services Division leadership regarding changes in the delivery of clinical care based on research and policy efforts. Supports YNHH management in their efforts to improve patient outcomes by providing necessary expertise and input.
11.3Represents Yale New Haven Hospital within the System and in the larger external community. 7.1. In conjunction with other clinical experts and divisional leadership, designs, develops, and implements patient care delivery methods for a defined patient population. Based on direct involvement in clinical practice and patient care, determines whether variation or less then optimal practice patterns exist and facilitates the introduction of best practices to improve outcomes. 7.2. Provides clinical consultation regarding patient care practices and advocates for changes in patient care processes and systems to facilitate optimal health care delivery. Formulates recommendations to Patient Services Division leadership regarding changes in the delivery of clinical care based on research and policy efforts. Supports YNHH management in their efforts to improve patient outcomes by providing necessary expertise and input. 7.3. Represents Yale New Haven Hospital within the System and in the larger external community.
BSN required, Masters in Nursing preferred
WOC certification from WOCNCB and or evidence of enrollment
Minimum of 2-3 years of clinical experience with wound management in an acute care setting. Experience with negative pressure therapy a plus
Registered Nurse in the State of CT
The nurse must possess good assessment skills, understanding of factors impacting wound healing outcomes, knowledge of evidence-based strategies and protocols for prevention and treatment of pressure ulcers, experience or willingness to learn VAC techniques, and to help manage ostomy and fistula patients to learn self care whenever possible. The nurse must be knowledgeable of YNHH performance improvement process, possess excellent customer service, phone, and interpersonal skills.
Interprets data, develop and implement therapeutic plans and follows through on the continuum of care of the patient. Implements plans through independent action, appropriate referrals, health counseling and collaboration with other health care providers.
