With the supervision of a Social Work Manager and the guidance of a Licensed Clinical Social Worker, the social
worker will perform professional social work services to Neonatal, Pediatric, Adolescent, Adult and Geriatric
patients/families/staff in the Yale-New Haven Hospital network. Professional supervision means face to face
consultation between one supervisor and one supervise consisting of review, periodic written evaluation, and
assessment of the supervisor's practice of social work for a total of 3,000 hours of work experience and 100 hours of
supervision as a requirement for licensure for Licensed Clinical Social Worker in accordance with the State of
Connecticut Regulations.

1. Social Work Clinical Practice:
Develop a full psychosocial crisis intervention assessment responsive to Neonatal, Pediatric,
Adolescent, Adult and Geriatric patient/family needs and concerns.

All patient care responsibilities will be performed consistent with age-specific criteria

Diagnose biopsychosocial functioning, disability and impairment for the Neonatal, Pediatric, Adolescent,
Adult and Geriatric patient.

Develop a treatment intervention plan responsive to the Neonatal, Pediatric, Adolescent, Adult and
Geriatric patient/family problems and formulate clinical impressions

Utilize, as a treatment modality, individual, couples, family, and/or group psychotherapy.
Provide information, consultation and professional support to interdisciplinary team members and
colleagues within the department.

Provide education of staff related to an individual and/or family's psychosocial needs.
Demonstrates initiative and leadership in developing services which respond to patient care demands.
Identify practice based continuous quality improvement.
Maintain an up-to-date, accurate and appropriate record of social work activities in patient's medical
chart which does not violate confidentiality.

Professional Development:
Attend educational seminars to maintain and meet expectations set forth by Hospital and departmental
standards.

Pursue additional opportunities for obtaining continuing education as set by department policy.
Involved in strengthening the community through participation in local, regional, state or national
activities.
Meet department/hospital expectations of growth goals set forth by the performance appraisal
document.

Accountability:

Accountable for following department/Hospital policy and procedures, NASW Code of Ethics, and any
other Hospital, governmental and/or regulatory policies, rules or regulations.

EDUCATION (number of years and type required to perform the position duties):
Master's Degree from an accredited School of Social Work.
EXPERIENCE (number of years and type required to meet an acceptable level of performance):
Minimum of two field placements from an accredited School of Social Work in an appropriate clinical setting.
SPECIAL SKILLS:
Work under the supervision of a Social Work Manager and the professional guidance from an LCSW and provide
clinical practice in a health care setting; to participate in teaching within the department, Hospital and university
setting. Computer experience and knowledge of word processing, spreadsheet and data entry systems.
ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, or
other areas of accountability):
Accountable for following hospital policy and procedures, NASW Code of Ethics, and any other governmental and /or
regulatory policies; will meet department/hospital expectations of growth goals set forth by the performance
appraisal document.

inherent in the responsibilities of this position):
In personal and job-related decisions and actions, consistently demonstrates the values of integrity (doing the right

Participate in Quality Improvement activities and departmental/hospital committees.
LICENSURE/CERTIFICATION:
Licensed Masters Social Worker (LMSW) from the State of Connecticut or those holding LMSW license from other
states will need an endorsement and meet the State of Connecticut license requirements.
