A registered professional nurse who utilizes the nursing process in providing nursing care and assumes the responsibility and accountability for prescribing, delegating and coordinating that care.
1. Demonstrates basic skill in assessment of health care needs. 1.1 Demonstrates basic skill in interviewing, observing and collecting health care data, performing systems assessment, identifying patient support system on admission according to hospital policy. 1.2 Identifies appropriate nursing diagnoses and expected outcomes based on ongoing assessment of patient health care needs. 1.3 Assesses patient health care needs, reflecting changes in patient health status in an ongoing manner. 1.4 Demonstrates assessment of patient health care needs on discharge. 1.5 Demonstrates assessment of patient health care needs on transfer/discharge to from unit. 2. Demonstrates basic skill in planning for patient health care needs. 2.1 Initiates clinical pathway utilizing admission assessment and guidelines for nursing care according to hospital policies. 2.2 Involves patient, family and/or significant others and members of the health care team in planning of nursing care and setting realistic outcomes. 2.3 Revises and individualizes clinical pathway to reflect change in patient condition. 2.4 Involves patient, family and/or significant others and members of the health care team in planning the discharge. 2.5 Develops and implements a teaching plan based on patient''s /family''s educational needs. 2.6 Involves peers and other health care team members in planning and revising the clinical pathway. 2.7 Effectively communicates with Case Manager regarding discharge planning activities for assigned patients. 2.8 Acts as patient advocate. 3. Demonstrates basic skills in implementing care to meet patient care needs. 3.1 Prioritizes patient care based on assessment. 3.2 Appropriately delegates tasks to team members of the patient care team. 3.3 Provides patient care based on nursing diagnosis and orders. 3.4 Demonstrates basic nursing skills appropriate to clinical area. 3.5 Demonstrates knowledge of test procedures and participates in patient education. 3.6 Provides health education to patient, families and/or significant others. 4. Documents outcomes appropriately, accurately and consistently. 5.1 Completes Admission Assessment Forms. 5.2 Maintains patient records, ie: transcription, CDS, pathways. 5.3 Accurately records patient data on flowsheets and MAR. 5.4 Performs discharge and transfer documentation completely. 5.5 Maintains unit-based statistical logs and information. 6. Demonstrates basic leadership skills and professional growth. 6.1 Identifies areas in performance needing improvement as evidenced by completion of self assessment skills checklist and ongoing communication with supervisor. 6.2 Sets specific realistic goals and target dates for completion. 6.3 Reviews and evaluates goals, as appropriate as discussed with supervisor. 6.4 Accepts constructive criticism and responds by implementing corrective action. 6.5 Communicates information through proper channels. 6.6 Utilizes correct resources for clarification of policy and procedure. 6.7 Demonstrates basic skills in acting as a resource person for members of the health care team. 6.8 Demonstrates appropriate knowledge and skill for the resource nurse role. 6.9 Attends and participates in a minimum of 8 staff meetings a year. 6.10 Understands and supports unit philosophy, goals, and objectives. 6.11 Fosters personal leadership development through participation in unit activities. 6.12 Acts as role model for PCTs and other members of the health care team. 6.13 Demonstrates sensitivity to diverse cultural needs. 7. Performs decentralized laboratory testing as credentialed and following established policies and procedures. 7.1 Appropriately collects specimens, performs test and interprets results. 7.2 Documents results and reports to physician as appropriate for patient care.
LICENSURE: Registered Nurse in the State of Connecticut. ACLS, TNCC and ENPC must be obtained within 18 months of employment and are offered through our system.
EXPERIENCE: One year of hospital RN experience preferred.
EDUCATION: Graduation from an accredited School of Nursing. BSN prefererred. Those with a Diploma or Associate's Degree need to show evidence that they are actively enrolled in a BSN or MSN program.
SPECIAL SKILLS: Good interpersonal skills. Successful completion of orientation.
