The Clinical Outcomes Leader (COL) is responsible for providing advanced clinical leadership and clinical outcomes management in the patient care setting and through the transitions of care. The COL facilitates evidence based care, improves systems processes, supports care coordination, facilitates outcomes management, establishes an evolving risk assessment, supports the patient experience, strives for ongoing quality improvement and serves as a role model for inter-professional communication. As a member of the unit leadership team, the COL promotes the value-based model among the nursing team as well for the continuum of care for patients.
Responsibilities: 1. Population Management Objectives 1.1. Evaluates the current status and needs of a given patient population 1.2. Consults with evidenced based literature and practice 1.3. Collaboratively establishes an evidenced based, evolving plan for the improvement of care 1.4. In coordination with other unit leadership, ensures that Patient Care Services staff are meeting evidence-based practice standards and institutional policies and strategies 1.5. Ensures appropriate patient care delivery methods for a defined population based on industry trends 1.6. Optimizes the utilization of resources to manage patient volume, decrease length of stay and drive down cost per case to improve efficiency 1.7. Addresses variations in clinical outcomes for the patient population. 2. Population Health/Accountble Care 2.1. Evaluates patients' pre-admission health status to begin to plan for discharge needs 2.2. Utilizes an interdisciplinary approach to decrease inter-facility readmissions to higher levels of care 2.3. Utilizes an interdisciplinary approach to decrease population focused patient readmissions to the hospital 2.4. Decreases unnecessary length of stay for a population 2.5. Collaboratively manages a data driven environment to measure unit outcomes for value-based purchasing measures, nurse sensitive indicators and all other initiatives identified by the organization that require monitoring 3. Outcomes Leader/Risk Analyst/Reduction and Elimination of Adverse Events 3.1. Establishes an evolving risk assessment methodology to evaluate, plan for and mitigate risk on behalf of patients, families, unit (care area) and organization 3.2. Decrease/Eliminate adverse events: i.e. HACs, CLABSI, CAUTI etc. 3.3. Utilizes information systems and technology that places knowledge at the point of care to improve health care outcomes 3.4. Reviews unit based data on nurse sensitive indicators for the patient population and compares them against both hospital, system and national benchmarks 4. Clinical Outcomes/Optimize Utilization of Resources 4.1. Oversees a patient centered environment at the point of care 4.2. Support locally and integrate with performance management charter teams 4.3. Unit focus on best practices, barriers to disposition, outpatient capabilities, provider variation in practice 4.4. Early intervention goal: Right Patient/Right Bed /Right Time patient surveillance 4.5. Collaboratively identifies and assures clinical measures that improve safety, timeliness, efficiency, effectiveness, and quality of care that are evidence-based 4.6. Evaluates the degree to which these clinical measures are patient centered 4.7. Collaborates with the interdisciplinary team to achieve all patient specific goals 4.8. Identifies future redesign projects that add value through improving cost and efficiency. 5. Synergy Leader 5.1. Collaborates with CNs, assuring patient care across the continuum 5.2. Serves as a mentor to clinical staff, empowering their advocacy of patient care 5.3. Promotes the used of scientific principles and critical thinking skills in the delivery of nursing care 5.4. Provides opportunities for staff to observe and participate in physical assessment, data review, evaluation of patient outcomes and other learning experiences/opportunities 5.5. Works with the staff on developing time management, prioritization and critical thinking skills on a continuum 5.6. Is a leader in, and facilitates staff engagement in innovation and evidenced based care 6. Clinical Leadership 6.1. Shares accountability for safe, effective, high quality care and optimal outcomes in the setting 6.2. Assimilates and applies research based information to design implement, and evaluate patient plans of care 6.3. Uses appropriate teaching/learning principles, strategies and materials as well as current research and technologies to facilitate the learning of the patient and all members of the care team 6.4. Practices and mentors skilled communication 6.5. Practices and mentors collaboration with interdisciplinary teams 6.6. Rounds daily with CNs at the point of care for coordination of services, education and quality assurance 6.7. Collaborates with shared governance and supports the submission of best practice changes 7. Customer Satisfaction 7.1. Supports the unit leadership team in ensuring that customer service standards reflect the service excellence pledge and policy 7.2. Models and upholds a customer service focus to all internal and external customers 7.3. Demonstrates value for all people in the work area 7.4. Empowers the patient and family to participate in care and decision-making 7.5. Provides or oversees the education of the patient and family to facilitate understanding of illness and self-care needs 7.6. Provides or oversees effective education of the patient and family in preparation for transfer or discharge 7.7. When appropriate, collaborates with consultative services for supportive care or intervention. Examples are, but are not limited to: Palliative Care, Social Services, Discharge Planning and Spiritual Service 8. Professional Member/Lifelong Learner 8.1. Baccalaureate degree required; Masters preferred. Would enroll and complete a Master's program. DNP, CNL, CNS or APRN preferred. Current CT state RN
licensure and/or eligibility for CT state RN licensure required
5 years NNICU clinical experience required, strong communication skills and strong interest in Evidenced based practice and Quality improvement
Current Connecticut State RN Nursing licensure and/or eligibility for CT RN licensure required. Relevant specialty certification required
Demonstrated ability to manage multiple priorities. Demonstrated clinical expertise, critical thinking and computer skills. Must have excellent prioritization and communication skills. Demonstrated ability to implement, support and effectively monitor change. Must be able to translate research and data into practice.

- Foundations of Safety, Quality and Performance Improvement
- Project management - (or Planning and managing Projects)
- Tools for Improvement - (or applying Lean Principles)
- Change Acceleration Process (CAP)
- Workout facilitation
- Collaboratively achieve defined unit-based targets, for example: patient experience, safe patient flow, reduction of HACs, population-specific LOS, etc.
- Accountable for safe, effective, high quality care and optimal quality outcomes in the setting.
- Identify additional projects which achieve YNHH approach to Clinical Redesign
- Authority to recommend changes to the patients care plan with the interdisciplinary team.
- Adds value through improving cost and efficiency.
- Represents YNHH and promotes the organization by adhering to and communicating the mission, vision, and values of the organization.
