YNHH Heart and Vascular seeks to achieve the highest level of safe and effective care as well as exceptional performance. Under the general supervision of the HVC Performance Improvement Associate Director , the incumbent will be responsible for coordinating
clinical information and data to support clinical quality performance measurement and improvement .Responsible to coordinating performance and safety improvement intatitives throughout all HVC areas. The incumbent ir responsible for tracking and monitoring the progress of all HVC improvement project. Responsible for working with teams and building consensus. Utilizing faciliation and team building skills to ensure acceptance.
The incumbent will support maintaining the accurate data collection, analysis and data dissemination. The
incumbent will coordinate performance improvement project under his /her supervision. The
incumbent will take personal responsibility for monitoring progress and updating all involved. The incumbent will work with
administrators, physicians, nurses and associated staff to ensure an efficient, accurate data collection process to
support performance improvement projects. Coordinate performance improvement and safety intitatives. Develop measureable outcomes.
Responsibilities: 1. Responsible creating coordinating and implementing safety and perfomrance activities for the HVC 2. Responsible for coding event reports using HRO categorization process 3. Responsible for facilation of meetings and setting up timelines for HVC and YNHH projects 4. Responsible for maintaining HVC dashboard updates and accuracy 5. Follows up on patient safety events and closes the loop with all involved 6. Able to present data and project progress through completion
BSN required.MSN preferred
CAP and Work out trained preferred. Experience with performance improvment activities
RN Licensure required
Excellent communcation skills. Knowledge of regulatory requriements for CMS/Joint Commission standards.Ability to coordinate and organize complex projects.
Patient oriented. Able to work independently .

Employee is accountable to the HVC Associate Director of Performance Improvement. Works indirectly with the HVC VP of operations and quality medical directors. Due diligence and personal accountability for all reports and data created. Transparency with data and reports is a must. Data and information created is reported publicly visible to all members of the HVC team, YNHH board and the public. Data and improvements related to this data could be reviewed and used by all regulatory agencies. Responsible development of measurement specifications and data integrity.
Managing multiple projects with interdisciplinary teams and timelines. Excellent organizational skills and time management. Understanding there are competing priorities and learning to manage them all. Must be able to negiotate educate and influence all roles within the institution about performance improvement and safety. Feedback and communication to all YNHH and HVC leadership. Able to accept constructive critism and amend improvement intiatives to ensure buy-in with out compromising integrity. Able to identify serious safety events and coordinate a root cause and apparent cause analysis. Mentor staff and leaders in HRO techniques
