Summary:
The Planning and Performance Consultant is responsible for collaborating within the team to identify and implement varied projects, programs and plans that support achievement of business unit goals. This individual will facilitate planning and performance tracking, and play an important role in maintenance and development of presentation materials for our various internal and external constituents. The Planning and Performance Consultant exemplifies the Standards of Professional Behavior aligned with our organizational values and commitment to cultural diversity.

Responsibilities:
  • 1. Support development of quantitative evidence of the accomplishment of system value based goals and objectives
  • 2. Maintain channels of communication between and among entities throughout NEMG/YNHHS which encourages interdisciplinary cooperation, respect, and mission values
  • 3. Collaborate with internal population health functional areas to optimize population health programs
  • 4. Perform operations budgeting including forecasting and financial results projection and analysis presentation
  • 5. Manage business planning and budget submission and management for a range of population health management programs and clinical integration activities
  • 6. Financially assess population health programs and evolution to risk based contracts
  • 7. Negotiate contracts with payers that include principles of value based care models and provide opportunities for system expansion with aligned incentives
  • 8. Assess parameters of payer contracts to ensure clarity of the models, metrics and calculations to support sound and informed decisions that drive value
  • 9. Develop and implement new and collaborative value based models.
  • 10. May specialize in one area of organization effectiveness (planning, project portfolio, or culture/change) or be assigned to directly support a business unit and ensures activities are aligned with corporate strategy.
  • 11. Identifies potential issues, project change, and scope data.
  • 12. Leads efforts to identify best practices.
  • 13. Identifies impacted parties, business partners, and resources required.
  • 14. Develops and designs processes and systems that support business needs.
  • 15. Designs methods for integrating functions and processes.
  • 16. Conducts cost/benefit analyses.
  • 17. Builds support for business change throughout the business unit.
  • 18. Provides process, project, and change management methodology coaching/consulting support to both local and enterprise wide initiatives.
  • 19. Leads the project prioritization and scheduling activities for the business unit.
  • 20. Provides expertise to the business areas in culture/change management activities.
  • 21. Provides expertise in process planning and portfolio management activities.
  • 22. Supports the execution of the operating plan.
  • 23. Promote the vision, mission, and values of Yale New Haven Health

    Other information:

    EDUCATION:

    Bachelor's Degree in health administration or a related field from an accredited school required. Master?s degree in healthcare administration, business administration, public health or a related discipline is preferred
    Six Sigma Black Belt preferred.

    EXPERIENCE:

    At least ten (10) years of related experience in healthcare administration or consulting. Experience may have been provider or carrier focused. Experience must include managing mid to large-scale change/project initiatives including knowledge of planning models and methodology, project management and change management experience (strategic and execution); or any combination of education and experience, which would provide an equivalent background.

    LICENSURE:

    Six Sigma Black Belt preferred.

    SPECIAL_SKILLS:

    Demonstrated experience developing and executing on complex project plans with diverse stakeholders; Ability to review workflows/ work processes and identify gaps (staffing, process, culture, etc.) Solid business acumen and analytical skills. Strong presentation skills, including both development and presenting. Strong verbal and written communication skills

    This position requires candidates to collaborate with numerous individuals to track execution of complex projects, implement process improvements, and optimize performance metrics and system initiatives.
