The primary function of the Clinical Nurse Practitioner is to provide comprehensive medical care with the collaborating physician of record. The APRN will work with the multidisciplinary team to provide optimum care for patients on their assigned service. Clinical responsibilities will be shared with resident team and include: evaluating patients, entering patient care orders, performing procedures, providing education, preparing patients for discharge and the communication and documentation of all patient care activities. The goal of the APRN role is to provide continuity of care in order to achieve optimal outcomes and patient satisfaction.
Responsibilities: 1. Assists with admission of patients. Perfoms and documents a complete history and physical examination in the EMR. 2. Order and obtain necessary diagnostic tests including imaging studies and laboratory tests. Follow through on interpretation of results and communicate with team and attending physician. 3. Request consultations from other services, and provide appropriate work up and follow through on recommendations, including communication to primary care physician. 4. Participate in daily rounds on primary patients and consults when appropriate. Assist with implementing managment plan as supervised by attendling physician and in collaboration with team. 5. Enter, renew, and update orders to include: rehabilitation services (physical, speech, and occupational therapy), IV therapy and medications, restraints, and discharge orders. 6. Be accesible, available, and visible to staff to address patient concerns. 7. Document pertinent information in the EMR. 8. Perform procedures of core priviledges. 9. Participate in on-call converage rotation as relevant to scheduled hours. 10. Assist with the evaluation and management of patients in the clinic setting. 11. Assist with procedures. 12. Produce discharge documentation including discharge instructions, completion of W-10s, dictation of discharge summaries. 13. Write prescriptions within scope of practice and confines of statute. 14. Participate in Quality Improvement and Performance Improvement activities. 15. Participate in special projects and YNHH initiatives at the request of Department Head, Manager, Section Chief, or Department Chair. 16. Participate in continuing education and maintain certification requirements. 17. Assist with orientation of new staff. 18. Act as clinical resource and participate in education of fellows, residents, and medical students. 19. Adhere to the spirit and intent of the "YNHH Service Excellence Pledge" and promote a positive work environment. 20. Provide specialty specific education to patients and families. 21. Participate in research projects as needed. 22. Provide formal and informal education for staff, patients, and families. 23. Serves as patient advocate and act as liason to patients and families. 24. Collaborates with multidisciplinary team (physician, nusing, rehabilitation services, religous ministries, social work, medical specialties) in developing a comprehensive care plan. 25. Communicates treatment plans to referring physicians, primary care physicians, care coordinators, social workers, and health care agencies as appropriate. 26. Demonstrate a working knowledge of diagnosis and treatment of chronic pain. 27. Ability to assist patients using interventional and non-interventional techniques. 28. Prior experience with pump refills. 29. Care of the patient with pain related clinical procedures.
MSN required. Graduate of accredited Nurse Practitioner program.
Three years of relevant experience is preferred.
Current CT state license as RN and APRN (or eligible). Certified as a Nurse Practitioner by the American Nurses credentialing Center (ANCC).
Excellent clinical, interpersonal and organizational skills. Ability to work independently. Ability to work with a diverse group of providers across the healthcare continuum.
Dual reporting structure with accountability to collaborating physician for medical matters and to Spine Center manager for administrative matters.
Evaluates, manages, and coordinates care in collaboration with the Attending physicians. May assist Attending physicians in most aspects of patient care. Works with Manager on special projects as assigned.
