LICENSED CLINICAL SOCIAL WORKER PART TIME - 24 HOURS PER WEEK PALLIATIVE CARE SERVICES PARK AVENUE MEDICAL CENTER, TRUMBULL, CT

Join our growing Palliative Care service in Southwestern Connecticut. This position will provide social work services in our brand new oncology focused outpatient clinic. You will join a respected Palliative Care service line as we grow to support our patient and family needs in the ambulatory setting. The new outpatient clinic has full interdisciplinary team support, including physician, RN care coordinator, chaplain, and practice supervisor.
This Part Time (0.6 FTE) LCSW role will include: \u2022 Perform intake and follow up social work assessments for clinic patients and families \u2022 Individual therapy for select patients and families based on needs identified \u2022 Assistance with coordination of social services as needed for patients, including applications and referrals for community based resources. \u2022 Bereavement screening and coordination with inpatient social worker and geriatric LCSW in the bereavement program. \u2022 Education of patients and families about goals of care and advanced care planning, including assistance with document completion.
The Section of Palliative Medicine works in close collaboration with a large Section of Geriatric Medicine, including a Geriatric and Palliative Care LCSW (outpatient) and Palliative Care MSW (inpatient). The section provides education to residents, fellows, nursing students, and social work students from many surrounding institutions including Yale. This team was recently recognized for the 2018 Empathy Amplified Award from the Cleveland Clinic for innovative programs and highly empathetic patient/family centered care Located in the beautiful New England setting of New Haven and Fairfield Counties, the area boasts wonderful family oriented residential communities with excellent school systems.
