Reports to the Director of Medicine/Ambulatory Services and works collaboratively with Program Director ,
physicians, Advanced Practice Providers (APP), and staff caring for patients to ensure goals and objectives are
successfully met. Responsible for collaborating with the Care Team at YNHH to promote high quality ,
comprehensive care to patients on the Service. Assists with program development, marketing and strategic planning associated with the clinical program. Assists Director with oversight of regulatory and clinical activity and ensures compliance with CMS, Joint Commission, state and national regulatory and clinical standards. Participates in
Hospital and Health System teams aimed at improving clinical care at YNHH and YNHHS. Facilitates engagement of physician and nursing leadership with strategic initiatives.
For this position, specific responsibilities will include: Responsible for evaluation and management of inpatient and ambulatory patients with chronic kidney disease, hypertension, and autonomic nervous system disorders. Experience in nephrology including hemodialysis and peritoneal dialysis preferred.
Responsibilities: 1. Maintains responsibility for all clinical aspects, initiatives, and responsibilities associated with the clinical
program. 1.1. Supports physician leader and Director with ongoing management of the clinical program. 1.2. Assists with clinical development, management, quality and outcomes of the clinical program. 1.3. Serves as a clinical leader and role model for the clinical program. 1.4. Maintains knowledge of, and provides evidence based medicine. 1.5. Maintains excellent understanding of medical knowledge regarding specified disease entities. 1.6. Provides accurate, comprehensive assessment of relevant medical history and condition, including
physical assessment and review of pertinent laboratory results, mental status, psychosocial support
and nutrition. 1.7. In collaboration with the medical team, coordinates the clinical care of patients. Completes a
comprehensive medical history and physical exam for all patients. Develops a comprehensive plan of
care and enters orders as appropriate in electronic medical record. Performs procedures related to
specialty and as credentialed. Interprets results and outcomes. 1.8. Assesses patient and family understanding of patient condition, goals of care and ongoing
management. Identifies health care needs and assesses health status of patient and family during
inpatient hospitalization and subsequent outpatient care as assigned. Monitors patients and develops
strategies to decrease length of stay and readmissions. Educates patient regarding treatment plans
and discharge care. 1.9. Provides patients and families with education regarding their disease process, medications, treatment
and plan of care. 1.10. Works with Care Management to ensure efficient transitions of care. 1.11. In collaboration with physician leaders assists with the development of best practices, standards of
care, benchmarks and treatment guidelines/goals that are evidence based and cost effective. 1.12. Communicates patient status, attends daily rounds when covering inpatient service and documents
progress in the electronic medical record. 1.13. In collaboration with the health care team, provides continuity of care to patients in the inpatient and
outpatient settings in order to achieve optimal patient outcomes and patient satisfaction. 1.14. Evaluates patient regarding discharge readiness. Communicates patient's treatment plan to referring
physician. 1.15. Coordinates activities within the Department to support research initiatives and ensure collaboration with
principle investigators. 1.16. Assists with coordinating the clinical activities related to the daily operations of the clinical service ensuring appropriate physician/APP coverage. 1.17. Oversees all aspects of the provision of care as outlines in the order sets, policies, procedures, and
educational materials in the electronic medical record. 1.18. Ensures that the clinical practice of specified care is aligned with the Joint Commission's and national
standards. 1.19. Will be responsible for coverage of specified clinical service Epic in-baskets. 2. Maintains responsibility for professional development. 2.1. Assists Director and Physician Leadership in the development of program goals and objectives. 2.2. Works with Leadership in collaborating with Finance, Information Systems and other departments to
manage individual billing practices and ensures appropriate charge capture. 2.3. Serves on hospital committees, attends seminars, conferences and professional meetings as required. 2.4. Represents the clinical program to all major constituencies including YNHH, Yale Medicine, clinical
departments, external agencies and organizations. 2.5. Ensures a professional work environment and maintains responsibility for professional practice and
individual professional development. 2.6. Provides on-call, weekend and holiday coverage as required. 2.7. Supports and exemplifies the principles of a High Reliability Organization. 2.8. Assumes responsibility for other duties as requested. 3. Assists with maintaining responsibility for quality improvement and regulatory aspects and initiatives
associated with the clinical program. 3.1. Maintains and prepares for compliance readiness related to ongoing Joint Commission and State
surveys and certifications. 3.2. Assists Leadership with clinical program quality improvement processes. Supports multidisciplinary
teams facilitating available methodologies to achieve established performance goals. 3.3. Participates in activities for effective review of patient care and operations. 3.4. Assists with coordinating and facilitating practice improvement opportunities related to patient care and
discharge planning. 3.5. Participates in quality improvement and customer service initiatives and provides education to clinicians
as required. 3.6. Participates in data collection and analysis related to the clinical program and assists with the
development of a clinical plan of action as required. 4. Provides leadership, expertise and support in planning, promoting, designing, and ensuring delivery of
education to nurses, physicians, support staff, patients, families and community based on best practice and
YNHH's strategic goals and initiatives. 4.1. Communicates and works with physicians and clinical staff to assess educational needs of patients ,
families and community. 4.2. Functions as a clinical resource for residents and nursing staff maximizing opportunities in the clinical
setting for informal education of staff, patients, and families. 4.3. Assesses needs and collaborates with physicians and clinical staff to develop and provide ongoing
education for staff. 4.4. Functions as a resource for providers and community agencies. 4.5. Coordinates and provides community education related to disease prevention and treatment.
Master's degree required. Graduate of an accredited Nurse Practitioner or Physician Assistant Program.
Minimum of two (2) years APP acute care experience required..
Current Connecticut APRN and DEA licenses required
Acute and/or ambulatory experience required with excellent clinical and decision making skills. Must possess
analytical skills to evaluate care needs, ability to gather relevant clinical data, and perform thorough physical
examinations. Must have ability to make independent judgments and implement required changes. Must be a
change agent and possess excellent interpersonal, communication, and leadership skills with good judgement and
tact when dealing with patients, families and clinical team.
Accountable to conduct accurate and appropriate evaluation, clinical care, and coordination of care of all assigned
patients. Assists leadership in assuming responsibility for the collaboration, integration, continued development, and
growth of the clinical program. Assists with implementing and monitoring process improvement while supporting the
department, programmatic, and institutional strategic plans.
