Evaluates, plans and implements specific treatment programs for patients. Collaborates with interdisciplinary treatment team members. Essential duties and responsibilities include the following. Other duties may be assigned.
Responsibilities: 1. Clinical Skills 1.1. Demonstrates Skill in Evaluation, Treatment Planning and Providing Therapy 1.2. Evaluates clients per department procedures as evidenced by random review of documentation in client chart. 1.3. Documents results of evaluation, formulates treatment plan, and identifies strengths, weakness and rehabilitation problems per departmental policies as evidenced by documentation in client chart. 1.4. Consistently formulates functional, measurable goals based upon evaluation results and client/family input as evidenced by documentation in client's chart. 1.5. Provides appropriate therapy to clients in accordance with evaluation results/recommendations as evidenced by supervisory observation and documentation in client chart. 1.6. Demonstrates knowledge of developmental stages when planning and implementing care for the person served. 2. Productivity 2.1. Manages a Full Caseload on a Daily Basis 2.2. Spends a minimum of 80% of time in direct hands on patient care as evidenced by random review of daily statistics. 2.3. Assumes other clinical responsibilities as assigned. 2.4. Consistently seeks out other professional duties. 2.5. Provides weekend and holiday coverage as per schedule. 3. Documentation and Administrative Tasks 3.1. Demonstrates consistency and timeliness with non-clinical tasks 3.2. Provides legible, concise and accurate documentation for patient care. Reports 100% of the time as evidenced by chart review. 3.3. Submits patient care reports, vouchers, time sheets, reimbursement reports and day off requests according to established department standards within established time frame. 4. Professionalism 4.1. Demonstrates Participation in Professional Development 4.2. Participates in team meetings, exchanges information/knowledge with staff, client, family members and others as appropriate. 4.3. Orients and educates students or volunteers regarding therapeutic services. 4.4. Keeps informed of current developments in the field by continuing education. 4.5. Has met requited contact hours. 4.6. Updates assessment and therapy procedures to integrate newly acquired information. 4.7. Provides effective inservices to colleagues, physicians, families and others. 4.8. Achieves all mutually established performance goals and objectives as indicated by supervisory review of annual goals. 5. Comprehensive Patient Care 5.1. Demonstrates Ability to Function as Interdisciplinary Team Member 5.2. Serves as contact person for family, referral source, funding source and other outside agencies. 5.3. Assures that discharge planning and arrangements for necessary follow up services are completed per department standards within established time frames. 5.4. Initiates interdisciplinary referrals when appropriate as determined by supervisory review of reports and feedback from other disciplines and utilization review. 5.5. Delegates and oversees patient treatment care plans for support staff (i.e. PTA, rehab tech, exercise physiologist, aides). 5.6. Evaluates and communicates findings and care plan. 5.7. Monitors that treatment plan is carried out per evaluation and notes are reviewed and co-signed regularly. 5.8. Effectively communicates with PTA and follows through with requests for change in treatment plan, re-evaluation of status and completion of discharge reports and notes.
Graduate of accredited physical therapy program or equivalent and B.S. degree
Clinical experience preferred. New grads are welcome.
Current Connecticut licensure in physical therapy or demonstrated eligibility.
All newly hired employees must have current BLS certification or obtain within one month of hire.
The worker must be able to stand, walk, talk, hear, lift, push, pull, reach, stoop, kneel, climb and perform repetitive motions, of the wrist, fingers or hands., The worker is subject to inside environmental/atmospheric conditions such as poor ventilation and is also subject to hazards, such as exposure to chemicals., The worker must be able to operate various exercise equipment, whirlpools, ultrasound and electrical stimulation equipment., The worker must be able to exert up to 100 lbs. of force occasionally, and /or up to 50 lbs. of force frequently and/or up to, 20 lbs. of force constantly to move objects or the human body., Functions with or without the assistance of reasonable accommodation as determined on a case by case basis.
