Specialty Clinical Pharmacist supports the vision, mission and goals of YNHH and the Department of Pharmacy and demonstrates proficiency in clinical pharmacist skills. Specialty Clinical Pharmacist must have met all general requirements of Clinical Pharmacist and Clinical Pharmacist 2 positions, in addition to specific pharmacy practice, drug information, educational/scholarly, and leadership requirements.
Responsibilities: 1. 1. Drug Therapy Management
1.1 In a timely fashion, reviews and verifies all medication orders to ensure safe, accurate, cost effective drug use.
1.2 Evaluates patient specific drug therapy management plans and therapy endpoints and suggests changes and improvements.
1.3 Understands PBMs and formulary requirements. Identifies and recommends alternative drugs for requests for non-formulary drugs required by insurances.
1.4 Performs pharmacist responsibilities as outlined by pharmacist protocols. (ie.renal dose adjustment protocol).
1.5 Consistently documents pharmacist interventions and follows through on process resolution
1.6 Consistently documents in Epic/QS/1, Therigy and communicates with physician's via Epic on patient specific issues. 2. 2. Interpretation of Clinical & Pharmacokinetic Lab Data
2.1 Demonstrates knowledge of pharmacokinetic and pharmacodynamic principles and applies to recommend changes in medication regimens.
2.2 Performs pharmacokinetic calculations. Recommends drug therapy changes to prescribers, and prospectively initiates appropriate dosage regimens.
2.3 Identifies targeted serum drug levels/lab values requiring pharmacist intervention
2.4 Evaluates the efficacy of drug therapy and anticipates side effects, toxicity or adverse effects.
2.5 Evaluates the effect of organ systems function on drug effect and recommends dose adjustments accordingly. 3. 3. Medication-related issue identification and resolution
3.1 Consistently documents in Epic and QS/1.
3.2 Identifies medication-related issues, communicates with proposed solutions to pharmacy leadership, and implements solutions.
3.3 Resolves medication-related issues by communicating with providers. Work to improve operational process through initiatives. 4. 4. Drug Information
4.1 Provides drug information to providers and paitents. Documentation of patient consultation at therapy initiation and follow-up, including review of medication effectiveness, side effects, lab monitoring, expected outcomes of therapy and duration of therapy.
4.2 Possesses comprehensive knowledge of drug information resources and applies information to specific patient cases.
4.3 Knowledge of policy and procedure for compliance with REMS program requirements.
4.4 Understands URAC accreditation along with observing policy and proceedures. 5. 5. Communication and Service Requirements
5.1 Understands role as integral member of the health care team
5.2 Demonstrates effective oral and written communication skills.
5.3 Able to effectively communicate information to patients based on patient specific age, language and literacy factors.
5.4 Evidence of patient education and counseling documented within an appropriate case management and EMR system. 6. 6. Education/Scholarly Activities
6.1 Serves as a primary preceptor for pharmacy students
6.2 Trains and mentors new pharmacists
6.3 Consistently attends intra-departmental pharmacy conferences
6.4 Presents journal club or case presentation at intra-departmental pharmacy meetings
6.5 Provides at least two inservices to pharmacy staff annually
6.6 Participates in appropriate education, training, and experience requirements for the specialty pharmacists positions
6.7 Completes annual Fraud, Waste, and Abuse training.
6.8 Completes pharmacy staff training/experience in managing specific disease states where they practice. 7. 7 Leadership
7.1 Consistently attends department meetings
7.2 Actively participates on a Pharmacy Department Committee
7.3 Participates in clinical, process improvement, cost savings, or safety projects
7.4 Identifies regulatory compliance issues and brings to attention of management 8. 8. Drug Distribution Activities
8.1 Acknowledge new prescriptions and refill requests as they come into the pharmacy
8.2 Prior Authorization process is consistently reviewed for appropriate processs
8.3 Reviews, adjudicate and delivers orders to patients that follow the process map developed
8.4 Responsible for recieving, documenting, investigating and responding to patient complaints
8.5 Participate in the process for medication error reporting
8.6 Follows regulatory standards surrounding 797 guidelines. 9. 9. The Specialty Clinical Pharmacist is responsible for overseeing the drug distribution activities and operations of the pharmacy to ensure patient and staff safety; the Specialty Clinical Pharmacist has the primary responsibility of:
i. Drug Therapy Management
ii. Interpretation of Clinical and Pharmacokinetic Lab Data
iii. Medication-related issue identification and resolution
iv. Drug Information
v. Communication and Service
vi. Education/Scholarly Activities
viii. Drug Distribution Activities 10. 10. The Specialty Clinical Pharmacist has the direct responsibility for the supervision of medication preparation and dispensing, evidenced by:
i. Maintenance of pharmacist to technician ratios that are in accordance with the Connecticut Commission of Pharmacy regulations
ii. Pre-verification of prescription prior to filling
iii. Verification of order after the prescription is filled
(number of years and type required to perform the position duties): Graduation from an accredited College of Pharmacy with a B.S, M.S or Pharm. D degree
Two years experience in Specialty Pharmacy preferred in the areas of case management, financial management, clinical monitoring, patient education, and knowledge in various disease states, including but not limited to Transplants, Oncology, HIV, Hepatitis C, MS and Pulmonary Hypertension. Must possess excellent customer service skills. Completion of an ASHP accredited PGY1 pharmacy residency preferred.
CT Pharmacist Licensure required, active and without restrictions. Board of Pharmacy Specialists Certification Preferred.
This individual should demonstrate in-depth knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution and reimbursement. Experience in Specialty Pharmacy drug utilization review and data analysis is highly desirable. Excellent written and verbal presentation skills are required. Must have expertise in patient management.
The individual in this position is held accountable to maintain the highest of professional standards and promoting rational and cost-effective therapy to positively influence patient outcomes. Successful or outstanding performance on all annual review goals and behaviors required. Must complete all hospital and department competencies and requirements for employement and maintain annual continuing education credits as mandated by the State of Connecticut Department of Consumer Protection.
The position requires leadership qualities and the ability to effectively communicate, both written and verbal. Must apply new ideas, knowledge and skill gained from experience and education to current situations; and should possess a superior knowledge of current standards of pharmacy practice. Must utilize sound clinical judgement in making therapeutic and technical decisions.
Travel to care sites for meetings may be required.
