The primary function of Physician Assistant (PA)/Nurse Practitioner (NP) working for the Hospitalist Service is to provide comprehensive care to medicine inpatients in collaboration with Attending physicians, PA/NP Manager, Director and Senior Manager of the Hospitalist Service. Each PA/NP has a designated supervising/collaborating physician.
Responsibilities: 1. Clinical Practice: 1.1. At the instruction of the PA/NP Manager and Director or his/her designee, assists in the care of medicine inpatients assigned to the Hospitalist Service, ensuring safe, high quality, patient-centered and efficient care. 1.2. Assists Attending physicians (Hospitalist Service or Private) with patient care. Primary responsibility for medical record documentation and related paperwork (except for discharge summaries), ordering of tests/procedures, communication with sub-specialty consults. 1.3. Identifies healthcare needs and assesses the status of medicine inpatients and coordinates post-discharge follow-up care 1.4. May assist with contacting patients' primary care physician 1.5. For new admissions, may obtain comprehensive health histories, perform complete physical examinations, complete accurate medication reconciliation, enter admission orders for appropriate laboratory and diagnostic imaging tests, and complete orders for therapy under the supervision of Attending physicians. 1.6. Rounds daily on assigned patients and has the primary responsibility for documenting the patient's status, data assessment and plan of care. Regularly communicates patient status information and test results to Attending physicians. 1.7. Identifies self in EPIC as covering provider by 8:30 am. Provide to the patient individualized business and/or picture cards and write your name daily on the patient's white board to identify the practitioner taking care of them on any given day. 1.8. Ensures all patient sign-outs are updated in EPIC for assigned patients by 5:30 p.m. 1.9. Identifies self to assigned patients and their families as the Hospitalist Service PA/NP responsible for their care. 1.10. Performs necessary invasive diagnostic and therapeutic procedures, as credentialed by Yale-New Haven Hospital Office of Physician Services or under the supervision of an Attending physician 1.11. Serves as a liaison and interacts with the rehabilitation staff, Food and Nutrition staff, and other ancillary services to ensure timely and comprehensive care for all patients. 1.12. Serves as a liaison to patients and their families 1.13. Educates patients and families about transition needs. Completes discharge paperwork, including the W-10 and prescriptions (but not the discharge summary). Reviews discharge instructions with patients and nursing at time of discharge. Coordinates outpatient follow-up. 1.14. Communicates patient treatment plans to social workers, care coordinators and health care agencies. Attend transition of care rounds daily when scheduled. 1.15. Promotes the dissemination of the newest clinical evidence in literature to improve patient care. 2. Education: 2.1. Identifies information needs of team members, patients and families. Provides related teaching and education to unit nurses and PA students from the Yale University School of Medicine Physician Associate Program. 2.2. Acts as a clinical resource for residents and fellows, and as a role model/preceptor for PA students. Maximizes opportunities in the clinical setting for informal education of staff, patients, and families. 2.3. Assists in orientation to new PAs/NPs and physicians working for the Hospitalist Service. 2.4. Actively researches information on Hospitalist Service patient medical conditions and shares information with other team members and students. 2.5. Participates in Hospitalist Service educational conferences 2.6. Participates in a level of continuing medical education (CME) activity annually that improves the quality of care provided by the Service. 3. Quality Improvement/Customer Satisfaction 3.1. Participates in team based quality improvement and patient/customer satisfaction initiatives. 3.2. Responds to patient complaints and reports patient satisfaction concerns to Patient Relations Department and PA/NP Manager, Senior Manager or Director so that appropriate action may be taken before patient discharge 3.3. Ensures medical record documentation meets YNHH, State and Federal guidelines, clearly documenting patient diagnosis and medical information required for legal and billing purposes. 4. Patient Safety 4.1. Immediately reports any concerns about patient safety to PA/NP Manager and/or Director. 4.2. Follows all Yale-New Haven Hospital patient safety policies and procedures for healthcare providers. 4.3. As a healthcare provider, is responsible for reporting adverse or potential adverse events related to patient care. 5. Operations 5.1. Follows all NEMG and YNHH policies and procedures. 5.2. Responds to pages in a timely manner and notifies Operations Manager if pager is lost or stolen. 5.3. Completes all mandatory training requirements related to HIPAA, Emergency Management, patient safety, or as assigned. 5.4. Ensures the required licensure and certifications are kept up to date and that NEMG always has copies of current licenses and certificates. 5.5. Bills for all procedures performed. Under signed name, writes Hospitalist or private attending name for day performed procedure. 5.6. Attends all team meetings, as required. 5.7. Dresses and acts professionally at all times. 5.8. Work hour responsibilities: Monday-Friday during the regular workday. Reports to work by 8:30 a.m. and expected to work a minimum of an eight and half hour day with pager coverage until 5:30 pm. Weekend and Holiday requirements are based on FTE status. Hours may vary as needed dependent on patient volume and acuity
Physician Assistant: Graduation from an accredited Physician Associate program required. Certified as a Physician Associate by the National Commission on Certification of Physician Associates.
Nurse Practitioner: Bachelor of Science or Bachelor of Art degree and graduation from an approved Nurse Practitioner program required. Certified as a Nurse Practitioner by the American Nurses Credentialing Center (ANCC) or another specialty nursing organization such as the Oncology Nursing Certification Corporation.
One (1) to two (2) years caring for medicine inpatients preferred.
Both hold a valid Connecticut medical license, Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification.
Excellent clinical, interpersonal and organizational skills. Ability to work independently with minimal supervision.
Accountable to conduct accurate and appropriate evaluation, care, and coordination of care of medicine inpatients.
Evaluates, manages, and coordinates care of medicine inpatients under the supervision or in collaboration with the Attending physicians. May assist Attending physicians in all aspects of patient care, including interactions with other services involved in the care of medicine inpatients, and possibly in the ED. Works with PA/NP Manager, Director and/or Senior Manager on special projects as assigned.
MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids.
Tdap and influenza vaccination are strongly encouraged.
