With supervision and/or consultation with a Clinical Social Worker or Social Work Team, the Crisis Intervention Worker provides social work services to the Neonatal, Pediatric, Adolescent, Adult and Geriatric patients/families/staff in the Yale-New Haven Hospital network.
Responsibilities: 1. Crisis Intervention Practice: 1.1. Develop a full psychosocial crisis intervention assessment responsive to Neonatal, Pediatric, Adolescent, Adult and Geriatric patient 1.2. Develop a treatment intervention plan responsive to Neonatal, Pediatric, Adolescent, Adult and Geriatric needs of patient/family 1.3. Provide information, consultation and professional support to the Interdisciplinary Team members 1.4. Develop a follow-up intervention for the patient and family's psychosocial needs. 1.5. Work collaboratively with the Social Work Team, as well as with other medical/nursing staff.
1.6. All patient care responsibilities will be performed consistent with age-specific criteria. 2. Professional Development: 2.1. Attend educational seminars to maintain and meet expectations of the Social Work Department 2.2. Pursue additional opportunities for obtaining continuing education set forth by the Hospital and departmental standards 2.3. Enhance their skills and knowledge through supervision 3. Accountability: 3.1. Accountable for following Hospital policy and procedures, as related to case management in the ER and documentation rules and regulations 3.2. The Crisis Intervention Worker will be accountable to appropriate social work management team members and will meet monthly with the team members.
Bachelor of Social Work or Bachelor's Degree from an accredited School of Social Work or equivalent.
Minimum of two years experience in a hospital, mental health, human service setting or an equivalent clinical setting.
Provide crisis intervention services in a health care setting. Strong interpersonal skills and ability to communicate in a crisis mode. Computer experience and knowledge of word processing, spreadsheet and data entry systems.
Accountable for following hospital/department policy and procedures, NASW Code of Ethics, and any other governmental and/or regulatory policies; will meet expectations of growth goals set forth by the performance appraisal document.
Participate in Quality Improvement activities and departmental/hospital committees.
