Physician Assistant - Bridgeport Hospital - Rapid Response Team
The primary function of Physician Assistant (PA) is to provide comprehensive care to patients in collaboration with physicians. Each PA has a designated supervising physician.

Responsibilities: 1. Clinical Practice: 1.1. Assists in the care of assigned patients, ensuring safe and efficient care. 1.2. Assists physician(s) with patient care, has primary responsibility for medical record documentation and related paperwork and ordering of tests/procedures and sub-specialty consultations. 1.3. Identifies healthcare needs and assesses the status of patients and coordinates post-discharge follow-up care. 1.4. May assist with contacting patients' primary care physician.For new admissions, obtains comprehensive health histories, performs complete physical examinations, enters admission orders for appropriate laboratory and diagnostic imaging tests, and completes orders for therapy under the supervision of Physician(s). 1.5. Rounds daily on assigned patients and has the primary responsibility for documenting the patient's status, data assessment and plan of care. Regularly communicates patient status information and test results to Physician(s). 1.6. Identifies self in CCSS (EMR/MR) as primary coverage person for assigned patients and updates pager list early in the day. Gives contact information to patients. 1.7. Collects all paperwork associated with the daily sign-out process and ensures that patient sign-outs are updated in CCSS for assigned patients by shift end. 1.8. Identifies self to assigned patients and their families as the PA responsible for their care. 1.9. Performs necessary invasive diagnostic and therapeutic procedures as credentialed or under the supervision of a physician. 1.10. Serves as a liaison and interacts with ancillary services to ensure timely and comprehensive care for all patients. 1.11. Serves as a liaison to patients and their families. 1.12. Educates patients and families about transition needs. Completes discharge paperwork when applicable, including the W-10 and prescriptions (but not the discharge summary). Reviews discharge instructions with patients and nursing at time of discharge. Coordinates outpatient follow-up. 1.13. Communicates patient treatment plans to social workers, care coordinators and health care agencies. 1.14. Shares the responsibility of carrying the coverage pager(s) on a rotating basis with other team members where applicable. 1.15. Promotes the dissemination of the newest clinical evidence in literature to improve patient care. 2. Education 2.1. Identifies information needs of team members, patients and families. Provides related teaching and education to unit nurses and PA students. 2.2. Acts as a clinical resource for residents and fellows, and as a role model/preceptor for PA students. Maximizes opportunities in the clinical setting for informal education of staff, patients, and families. 2.3. Assists in orientation to new staff working. 2.4. Actively researches information on patient medical conditions and shares information with other team members and students. 2.5. Participates in educational conferences and attends Grand Rounds, as time permits. 2.6. Participates in a level of continuing medical education -CME activity annually that improves the quality of care provided. 2.7. Quality Improvement/Customer Satisfaction 2.8. Participates in team based quality improvement and patient/customer satisfaction initiatives. 2.9. Responds to patient complaints and reports patient satisfaction concerns to Patient Relations Department at local delivery network and manager so that appropriate action may be taken before patient discharge. 2.10. Ensures medical record documentation meets NEMG, State and Federal guidelines, clearly documenting patient diagnosis and medical information required for legal and billing purposes. 3. Patient Safety 3.1. Immediately reports any concerns about patient safety to manager and/or Adminstration. 3.2. Follows all patient safety policies and procedures for healthcare providers. 3.3. As a healthcare provider, is responsible for reporting adverse or potential adverse events related to patient care. 4. Operations 4.1. Follows all NEMG policies and procedures. 4.2. Responds to pages in a timely manner and notifies manager if pager is lost or stolen. 4.3. Completes all mandatory training requirements related to HIPAA, Emergency Management, patient safety, or as assigned. 4.4. Ensures the required licensure and certifications are kept up to date and that NEMG always has copies of current licenses and certificates. 4.5. Bills for all procedures performed and documented. Under signed name, writes Hospitalist or private attending name for day performed procedure. 4.6. Attends all team meetings, as required. 4.7. Dresses and acts professionally at all times.
Graduation from an accredited Physician Associate program required. Certified as a Physician Associate by the National Commission on Certification of Physician Associates. Both hold a valid Connecticut medical license, Connecticut Controlled Substance Registration and Federal Drug Enforcement Agency (DEA) Certification.
One (1) to two (2) years of inpatient experience preferred.
Excellent clinical, interpersonal and organizational skills. Ability to work independently with minimal supervision.
Accountable to conduct accurate and appropriate evaluation, care, and coordination of care of patients.
Evaluates, manages, and coordinates care of patients under the supervision or in collaboration with the Physician(s). May assist Physician(s) in all aspects of patient care, including interactions with other services involved in the care of patients, and possibly in the ED. Works with Management on special projects as assigned.

MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids.
Tdap and influenza vaccination are strongly encouraged.
