Clinical Supervisor, Outpatient - Mystic, CT

Summary:
To provide professional mental health services to patients and their families or significant others at all levels of care and locations required to meet community needs. Oversees Ambulatory (Outpatient) services including the supervision of Ambulatory clinicians and clinical staff under the direction of the Chair of Psychiatry and in collaboration with NEMG operations. Works with the Chair of Psychiatry and with the psychiatrists and APRNs working in the Ambulatory setting to ensure quality patient care services are provided. Routinely provides clinical care in the Ambulatory setting and elsewhere when directed by the Chair of Psychiatry.

Responsibilities:
  • 1. Supervisor Responsibilities
  • 1.1. Oversees components of service delivery and the maintenance of ethical and exemplary ambulatory services and patient relations
  • 1.2. Maintains monthly patient lists and clinical operations data to organize and analyze for use by the Chair of Psychiatry
  • 1.3. Oversees Quality Improvement activities including preparation of required reports, collection of program service data, and audit preparation
  • 1.4. Works with other members of Ambulatory services to assure successful communication and coordination between all Yale New Haven Health programs
  • 1.5. Assumes responsibility for promoting quality patient care throughout the department by working with the Chair of Psychiatry and the clinicians and staff in other service areas within the department
  • 1.6. Understands all policies and procedures of Northeast Medical Group (NEMG) and Yale New Haven Health System
  • 1.7. Works effectively with billing staff and Ambulatory services clinicians to ensure efficient and accurate billing of Ambulatory Services
  • 1.8. Reviews and discusses patient complaints with the Chair of Psychiatry, Ambulatory services prescribers, Ambulatory services clinicians, and patient relations staff as appropriate. When appropriate, directs corrective action(s) and coordinates a collaborative response to the patient with patient relations
  • 1.9. Works collaboratively with local practice managers to coordinate schedules and space for Ambulatory clinical services
  • 1.10. Regularly edits and approves Kronos/payroll systems for Ambulatory services staff
  • 1.11. Provides regular input with regard to planning and resource allocation for Ambulatory services
  • 1.12. Coordinates, approves, and tracks requested time off for Ambulatory services clinicians and ensures that NEMG operations receives accurate time off and coverage information
  • 1.13. Provides coverage as necessary and possible for the patient care responsibilities of other Ambulatory services clinicians in instances of PTO, call outs, and/or sick calls
  • 1.14. Works with the Chair of Psychiatry and NEMG operations to meet financial goals through considerations of staffing, clinical practice, and billing
  • 1.15. Ensures full compliance with HIPAA and all other regulatory agency requirements
  • 1.16. Identifies opportunities for quality improvement and initiates appropriate referrals
  • 1.17. Coordinates orientation and training for new employees working within Ambulatory services
  • 1.18. Ensures that Ambulatory services clinicians maintain yearly professional educational obligations and forwards certification according for the respective educational credits received
  • 1.19. Provides leadership, support, and resources to Ambulatory services clinicians and other clinical staff
  • 1.20. Works with the Chair of Psychiatry, NEMG operations, and Ambulatory services prescribers to provide monitoring, oversight, and evaluations of Ambulatory services clinicians and other clinical staff. Provides coaching counseling, and other forms of support or mentoring to staff to promote them attaining their performance goals
  • 1.21. Coordinates the placement of clinical trainees or interns in the appropriate supervisory environment
  • 1.22. Assists the Chair of Psychiatry with organizing and preparing for monthly meetings of Ambulatory services staff
  • 1.23. Leads by example, successfully demonstrating the personal attributes defined in the Yale New Haven Health System Standards of Professional Behavior, Promotes an atmosphere where staff are encouraged to express ideas and contribute to the improved functioning of the Department
  • 2. Clinical Responsibilities
  • 2.1. Participates in patient assessment prior to and upon completion of treatment.
  • 2.2. Assesses and documents patient status/progress at appropriate intervals in compliance with policy.
  • 2.3. Shares signification assessment and treatment information with other team members through appropriate, timely and clear communication.
  • 2.4. Work collaboratively with primary care practice Physicians, Medical Assistants and Nurses in the various treatment and practice settings.
  • 2.5. Provides appropriate individual, family, couples and/or group psychotherapy to patients on the basis of an interdisciplinary plan.
  • 2.6. Participates in the formulation and implementation of an individualized interdisciplinary treatment plan based on a thorough assessment of the patient?s problems and needs.
  • 2.7. Conducts psycho-diagnostic and disposition intake/screening and crisis evaluations of patients.
  • 2.8. Assists psychiatric medical staff in monitoring the use, effectiveness, and side-effects of psychiatric medications.
  • 2.9. Demonstrates basic clinical and technical skills according to established standards of practice.
  • 2.10. Documents all significant interventions in appropriate records including the electronic medical record.
  • 2.11. Actively participates in patient and family/significant other(s) education.
  • 2.12. Demonstrates an ability to establish and maintain meaningful therapeutic relationships with patients.
  • 2.13. Effectively manages crisis/situations/routine treatment interactions with due respect for patient rights, legal/ethical obligations, and the therapeutic goals of any interventions used.
  • 2.14. Conducts and documents regular reviews of treatment effectiveness/patient progress, and psychosocial assessments.
  • 2.15. Prepare probate court documents and attend probate proceedings when appropriate and necessary.
  • 2.16. Contacts mental health agencies and refers patients for case management, residential and community based step-down brief care services.
  • 2.17. Contacts treatment providers involved in patient care and obtains collateral data as well as supplies information on patient care as needed.
  • 2.18. Assists with the Direct Admission Screening process.
  • 2.19. Meets performance expectations for Customer Service, Teamwork, Resource Utilization, and Staff and Self Development as outlined in the performance review.
  • 3. Ambulatory Services
  • 3.1. Provides evidence-based psychotherapeutic treatment services in a community-based practice, including providing a range of individual, group, couples and family psychotherapies, on a short-, intermediate-, or long-term basis as appropriate.
  • 3.2. Provides or assists their psychotherapy patients in obtaining referrals for higher levels of care, alternative care, supplemental care, or support services as appropriate
  • 3.3. Works collaboratively with psychiatrists and other psychiatry providers in the practice in the assessment and treatment of outpatients.
  • 3.4. Works collaboratively with primary care physicians and other allied providers to improve the care of patients served in the community-based practice; provides peer support and education when possible with regard to the assessment and management of patients with psychiatric illnesses and/or emotional or behavioral problems.
  • 3.5. Performs other duties as assigned or directed by the Chair of Psychiatry to ensure the smooth operation of Ambulatory Services and/or the Department.

    EDUCATION:

    M.S.W., M.A., M.S., or M. Ed.

    EXPERIENCE:
    Two (2) years clinical experience

    LICENSURE:
    Licensed as one of the following: LCSW, LPC, LMFT, APRN. Current CPR certification

    SPECIAL_SKILLS:
    Knowledge of medical equipment and instruments used to administer patient care
    Knowledge of common safety hazards and precautions to establish/maintain a safe work environment Knowledge of health promotion, health risk identification, and patient education principles and techniques Develop and use skills in the implementation and use of the electronic medical record (EMR) applications. Ability to analyze complex data and draw conclusions
    Ability to interpret, adapt, and apply guidelines and procedures
    Ability to establish/maintain effective working relationships with patients , clinical staff, and the public
    Population expected to work with include Pediatric 1-12, Adolescent 12-18, Adults 19- 64 and Geriatrics 65 + years of age.
