Summary:
A Professional Registered Nurse who is accountable for the planning and administering of direct nursing care to patients in the Operative Suite, and in accordance with the American Nurses Association Standards of Professional Nursing Practice. The Association of peri Operative Room Nurses (AORN).

Responsibilities:
  • 1. Clinical Competence in Patient Care
  • 1.1. Complies with patient safety standards.
  • 1.2. Provides individualized, goal directed care to patients with consideration of the aging process of populations served.
  • 1.3. Collaborates with other members of the patient care team to evaluate patient out comes and determine alternatives as needed.
  • 1.4. Demonstrates knowledge and skill of all equipment used in the operation suite.
  • 1.5. Assesses patients and themselves by following guidelines for infection control and protective equipment.
  • 1.6. Demonstrates knowledge + ability to clean + disinfect equipment.
  • 1.7. Appropriately sets up OR procedures using physicians case cards + sterile techniques.
  • 1.8. Reports pertinent clinical data to the physician.
  • 1.9. Accurate identification of correct surgical site + patient using the Universal Protocol.
  • 1.10. Performs and documents the pre-operative nursing assessment with attention to pre-op lab findings, physical findings, psychological data, educational needs and significant person/family support.
  • 1.11. Clearly explains the operative procedure. Communicates concerns about patient to appropriate health care provider.
  • 2. Safety Administers Medications and IV Therapies
  • 2.1. Adheres to the 5 R?s of medication administration. Is proficient at using the Pyxis for medication withdraw.
  • 2.2. Is proficient at pyxis inventory and correct procedure for discrepancy documentation.
  • 2.3. Follows correct procedure for administration/documentation and waste of controlled substances, thru the pyxis system.
  • 3. Professional Behavior and Growth
  • 3.1. Demonstrates evidence of professional growth by education thru attendance at seminars and in-services related to field of practice, and on hospital and unit committees.
  • 3.2. Researches unfamiliar procedures, diagnosis, medications, and shares findings.
  • 3.3. Acts as a resource to staff, patients and their families/significant others. Orients new employees.
  • 3.4. Keeps current with all on line mandatory education, thru the intranet, under mandatory education and thru education sent by Dept. Manager on Hospital e-mail.
  • 3.5. Attends all required safety training programs and can describe his or her responsibilities related to safety of the organization and specific job related hazards.
  • 3.6. Knows it is there Responsibility to keep Licensure and certifications current and keep manager updated on changes of before mentioned.

  • 7.2. Responds to customer?s issues/requests in a clear, concise, and timely manner utilizing active listening skills and validating information.

  • 7.4. Maintains confidentially with respects to patient, co-workers and hospital information by disclosing information only on a ?need to know basis? and discussing information in appropriate areas. Follows HIPPA guidelines in all communications regarding patients.
  • 8. Circulator
  • 8.1. Identify needs supplies and equipment to prepare OR Suite for patient care; inspect OR Suite to determine if it is clean and equipment is in readiness for case.
  • 8.2. Determine the patient?s chart is complete and maintain accurate records of all preoperative patent care following established policies and procedures.
  • 8.3. Initiate and maintain correct sponge, needle and instrument counts throughout preoperative period.
  • 8.4. Prep the incision site according to the established procedure.
  • 8.5. Correctly identify, label and Log specimens to the Path Lab. Making sure the correct patient name, correct specimen name and correct side is listed on all documents.
  • 8.6. Remain at patient?s side and assist Anesthesiologist as required during induction and emergence from anesthesia. Coordinate patient?s transfer, establishing and maintaining correct body alignment and using Safe Lit devices.
  • 8.7. Prepare equipment and supplies for use in the procedure. Determine room is in readiness and OR Suite is stocked for case.
  • 9. Scrub
  • 9.1. Scrub, gown and glove self and other team members for the procedure.
  • 9.2. Count sponges, needles and instruments with RN Circulator according to established procedure.
  • 9.3. Handle instruments and supplies correctly and ensure their proper functioning, before, during and often the case is over. Notify MD if something is broken.
  • 9.4. Anticipate surgeon?s need, and request additional items from RN Circulator in sufficient time for surgical team?s use.
  • 9.5. Label all solutions/medications as they are added to the sterile field.
  • 9.6. Assist with sterile table and room preparation, and clean-up following the procedure. Take responsibility of contaminated materials, and dispose of sharps and needles in correct containers.
  • 9.7. Notify Circulator of any changes to Surgeon?s Case card for editing after the case is over.
  • 9.8. Direct and assist with between case clean up and proceed with preparation for the next case, including but not limited to Instruments, Trash, Suction canisters and Equipment.
  • 10. PACU
  • 10.1. A. Airway management
  • 11. Assess rate, depth, symmetry of respiration
  • 12. Auscuifate chest
  • 13. Provide adjunct airways and supplemental oxygen as indicated
  • 14. Place pt. on cardiac monitor
  • 15. Identify cardiac rhythm
  • 16. Report and treat dysrythmias as indicated
  • 16.1. C. Surgical site assessment
  • 17. Observe op site ? note integrity of dressing and/or drains
  • 18. Report abnormal/unexpected changes to surgeon
  • 18.1. D. Prepares for and assists with other procedures, to include but not limited too:
  • 19. Elective cardio versions
  • 20. Nerve blocks
  • 21. Prep?s and hair removal
  • 22. Lidocaine infusions
  • 23. Responds to met codes
  • 24. Assists with Cataract and Minor Ortho cases in the Phase 2 area.
  • 25. Rotates to ACU as assigned for Phase 2 recovery.
  • 26. Stays current on McKesson Applications, including but not limited to (clinical care station, Med. Admin., Patient supply charging, HPF and ORM) as they pertain to PACU documentation.
  • 27. Assists in transporting patients back to inpatient area?s.
  • 28. McKesson/ Paragon Applications for all RN?s
  • 29. During periods of room downtime (no cases) log onto ORM test and practice issues that you are not comfortable with or don?t do every day.
  • 30. Has the ability to log into ORM and into correct patient for intra op charting.
  • 31. Is able to navigate the electronic record freely, with interruptions and doesn?t have undue delays.
  • 32. Enters all data appropriately and correctly, including but not limited to ( Correct ? Participants, Times, Care Plan, Time Out, Wound Class, Medication times) and closes out the record before getting out of it.
  • 33. Adds remarks to the paper copy of the case card about items or changes, for later editing.
  • 34. When on call can correctly add the case to the system with the correct patient and visitor id and correct procedure or do a new/from if one isn?t available.
  • 35. Drops unused items down off of the case card as unused or wasted.
  • 36. Knows how to troubleshoot issues during documentation or knows who to call if you can?t.


    Other information:

    EDUCATION:

    Graduate of an accredited school of nursing


    EXPERIENCE:

    2-3 years med/surg experience preferred or previous experience in the operating room environment.


    LICENSURE:

    Must have and maintain current licensure in the State of Rhode Island. RN, BLS Certification and must obtain ACLS certification.
