The Nurse Practitioner works collaboratively with the health care team. He/she will assess, plan, implement and
evaluate care for patients. He/she will, in collaboration with other team members provide programs, treatment and
counseling appropriate to the patient's plan of care. He/she will consistently demonstrate proficiency in working with
the designated patient population, and serve as a unit resource and teacher.
FOR THIS SPECIFIC POSITION:
Provider will rotate between Stamford outpatient and New Haven locations. Should have experience with renal disease, dialysis and working with pre-transplant patients.

6. Responsible for promoting the Hospital Patient/Guest Relations program.
7. The Nurse Practitioner must be able to demonstrate the knowledge and skills necessary to provide care
appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate
knowledge of the principles of growth and development over the life span and possess the ability to assess
data reflective of the patient's status and interpret the appropriate information needed to identify each patients
requirements relative to his or her age-specific needs, and to provide the care needed as described in the
MSN Required, Graduate of Nurse Practitioner Program. Current CT APRN License.
3-5 years experience in area of specialty preferred.

Licensed as a NP, Certification in area of specialty; Basic Life Support (BLS)
