Communicates and follows-up with patients, physicians, internal departments and external sources/providers as necessary to ensure smooth and efficient patient flow throughout the pre and post surgery process. Triages needed preoperative consults and referrals from patient history and/or physical examination. Performs various duties including counseling patients on financing options, scheduling appointments and tests, obtaining patient health history information, etc. Reviews all preoperative clinical information and results from patient consults. Tracks patient status and all other requirements using tracking tool software program and checklists.
Responsibilities: 1. Bariatric Patient Flow. 1.1. Manage patients' status to ensure patients move through the process from seminar to surgery as efficiently as possible. 1.2. Facilitate evening and off hours seminars at various geographic locations as assigned. 1.3. Manage the assignment of patients without a surgeon preference. 1.4. Be knowledgeable of insurance requirements for authorization for all payors, and facilitate preauthorization process. 1.5. Assist with registration of new patients for seminars and collect all pertinent demographic data; including referral source. Document all information in the tracking tool. 2. Patient Tracking 2.1. Develop expertise on diet history, medically supervised weight loss, and psychological evaluation requirements. 2.2. Triage patient intake history forms and outline needed preoperative appointments and consults, including those to meet insurance requirements, and any additional needed as indicated by patient history. 2.3. Review incoming clearances and diagnostic tests in a timely manner, and document in tracking tool/checklist to monitor patient progress in meeting insurance and clinical requirement. 2.4. Assist with scheduling of appointments as needed. 2.5. Report pipeline status to the Program Manager, and surgeons on a monthly or as needed basis. 3. Collaboration with Administrative Staff 3.1. Provide guidance on contacting external providers to obtain and collate missing paperwork on behalf of patient. 3.2. Faciitate follow-up by administrative staff with patients no less frequently than monthly or as frequently as necessary to check status of requirements fulfillment as a means to keep patients engaged with program. 3.3. Review patient tracking with administrative assistants and provide guidance on patients readiness for surgery. 3.4. Review all patient workflows for completeness prior to submission for insurance authorization. 4. Professional Behavior 4.1. Attend staff meetings and multidisciplinary meetings as requested. 4.2. Maintain patient confidentiality. 4.3. Performs other duties as necessary or directed by the Program Manager 4.4. Maintain professional demeanor at all times with patients, providers, and staff. 5. Multidisciplinary Collaboration 5.1. Participates as active member of the multidisciplinary bariatric team, leads team, as appropriate, in forums discussing patient care. 5.2. Conducts patient teaching in collaboration with other multidisciplinary team members, in both individual and group settings. 5.3. Collaborates with Surgeons/PAs/APRNs in evaluating the outcomes of all program patients. 5.4. Reviews abnormal consultation results with surgeons/PAs/APRNs as indicated. 5.5. Facilitate and ensure that all services necessary are coordinated within organizations different departments, and externally as needed. 6. Medication Administration. 6.1. Clarifies any questions regarding physician prescriptions and preoperative plan of care with provider. 6.2. Prepares and administers medications to patients according to the departmental policy, ensuring safe and
accurate delivery of medication 100% of the time. 7. Team Participation 7.1. Participates in interdisciplinary conferences. 7.2. Contributes to a positive work environment by being responsive to the need for change. 7.3. Participates in activities to improve care based on quality indicators. 7.4. Communicates with all members of the care delivery team in an accurate, timely and open manner. 7.5. Maintains a collaborative work relationship with staff members through respect and recognition of
cultural and human diversity. 7.6. Utilizes resources in a fiscally sound manner. Identifying opportunities to improve and standardize to
reduce cost. 7.7. Communicates in a calm, professional manner with patients, families, members of the care team and
other health care personnel. 7.8. Accepts and elicits feedback and works towards resolving problems and clarifying issues and
concerns in a constructive manner. 8. Clinical Competency 8.1. Attends all mandatory and continuing educational classes. 8.2. Participates in patient care area, department and division continuing education programs. 9. RN Duties in Ambulatory Setting 9.1. Assists with all duties in care of patients in clinic as needed. 9.2. Triages phone calls to the clinic regarding preoperative bariatric patients, and handles follow up phone calls to patients. 10. Quality Improvement Activities 10.1. Participate in data collection relating to clinical and operational efficiencies. 10.2. Support any marketing and community outreach efforts. 10.3. Tracks and monitors patient flow to ensure patient through put is not compromised. 10.4. Identifies processes or areas where bottlenecks occurs, and offers solutions for process improvement.
RN required, B.S.N. degree preferred.
Minimum of two (2) years RN experience required. Experience working with Bariatric patients preferred.
CT Registered Nurse Licensure
Knowledge and skill in the use of computer systems and software including Microsoft Office Products (Word, Excel, Power Point, etc.). Strong interpersonal skills to effectively and accurately communicate with patients, physicians and related clinical staff. Strong organizational skills to accurately keep track of patient status. Ability to review and interpret lab and test results. Time management and multi-tasking ability to monitor numerous patients simultaneously. Ability to function well as an integral member of a multidisciplinary team and promote cooperative teamwork. Ability to lead multidisciplinary patient review meetings.
