Summary:
Working collaboratively as a member of a multidisciplinary treatment team, the Outpatient Psychiatric Counselor works collaboratively with the licensed staff on the ongoing development of the milieu and activities that promote quality patient outcomes.
Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based on the 4 principles of PFCC: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence by creating a great "First Impression" by demonstrating exemplary customer service skills for all customer groups including patients, families/friends, physicians, staff and support department personnel.

Responsibilities:
  • 1. Assists staff in maintaining a therapeutic milieu/environment to ensure the care and treatment of patients.
  • 1.1. Acts as a co-facilitator in select therapeutic groups.
  • 1.2. Participates in the ongoing development of the therapeutic milieu/environment, creatively shapes milieu activities to the needs of the current patient population's developmental needs.
  • 1.3. Engages in anticipatory planning and problem solving in collaboration with the clinicians and MD/LIPs within the milieu/environment.
  • 1.4. Initiates patient activities and social interactions in individual and group situations consistent with the overall interdisciplinary treatment plan.
  • 1.5. Provides environmental and emotional safety for each patient through consistent limit setting, boundary management, and monitoring of overall activities and interactions of patients.
  • 1.6. Maintains a clean and safe patient environment.
  • 1.7. Collaborates with the licensed clinical staff in co-facilitating therapeutic groups; attends supervision as scheduled to increase understanding of co-facilitator relationship and patient responses to groups.
  • 2. Provides individualized patient care to insure the safety and dignity of the patient.
  • 2.1. Collaborates with members of multidisciplinary team offering suggestions and recommendations on addressing patient/family needs.
  • 2.2. Follows the patient care plan preserving patients' safety, dignity and autonomy.
  • 2.3. Obtains specimens while preserving patient's safety, dignity and autonomy.
  • 2.4. Collaborates with clinicians and MD/LIPs to assist in the development and ongoing evaluation of the patient's plan of care and development of discharge plan.
  • 2.5. Observes and communicates behavioral responses to interventions to treatment team in clear, concise, and timely manner.
  • 2.6. Notifies the patient's treatment team of any changes in the patient's physical/ psychiatric condition and requests assessment by the licensed clinical staff/MD/LIP.
  • 2.7. Conducts other clinical care activities within allowable scope under direction of Program Manager.
  • 3. Participates in program activities that promote a cohesive staff group.
  • 3.1. Participates in interdisciplinary staff meetings and if unable to attend, reads and signs minutes.
  • 3.2. Participates in program based activities that have a positive impact on unit practice (e.g., collaborates with licensed staff on inservice presentations, policy development task force, etc.)
  • 3.3. Maintains all competencies and regularly attends in-services relevant to field.
  • 3.4. Attends supervision as scheduled and seeks supervision from clinicians, Program Manager as needed.
  • 4. Meets Departmental Goals
  • 4.1. Collaborates with all staff to help achieve annual Departmental Goals
  • 5. Assists staff in maintaining a therapeutic milieu/environment to ensure the care and treatment of patients.

    Other information:

    EDUCATION:
    Associates degree in Psychology or related field, Bachelors Degree preferred, or equivalent combination of education and experience.

    EXPERIENCE:
    Minimum two (2) years prior experience in Psychology and or related field, working with specialized populations such as emotionally disturbed adults, adolescents and or children and families. Experience working with adolescents in a psychiatric setting preferred.

    SPECIAL SKILLS:
    Ability to work collaboratively in an interdisciplinary team to communicate and effectively deal with complex effectively charged situations and possess group facilitation and crisis management skills.
