The RN Case Manager provides and ensure competent and safe delivery of patient care in home environment.

Responsibilities

  • 1. Assess patients (physical, psychological and social) up to level of competency, including all factors relevant to patient/family safety, need and quality of care;
  • 2. Assess needs of the patient/family for other home health services and refer as appropriate;
  • 3. Assess/determine aspects of care plan for delegation to home health aides;
  • 4. Assess appropriate payment source for home care services and obtain ongoing authorizations as needed;
  • 5. Admit patients to HCP and notify/conference with physician to develop Care Plan;
  • 6. Schedule patients according to Care Plan. Make modifications as needed;
  • 7. Ensure authorizations and followed. Provide information to payors on prompt basis;
  • 8. Initiate/participate/document case conferences;
  • 9. Instruct/document patient/family teaching;
  • 10. Assess patient progress and take appropriate action when change in patient condition occurs;
  • 11. Coordinate services with the patient, family and others involved in patient care;
  • 12. Conduct pre-discharge planning and notify physician as goals are met;
  • 13. Identify/train patient/family on special equipment/supply needs; and
  • 14. Perform other job duties as requested.

Qualifications

EDUCATION


RN with active CT nursing license;


EXPERIENCE


Minimum of one (1) year home health or hospital nursing experience;


LICENSURE


RN with active CT nursing license; Current CPR certification


SPECIAL SKILLS


Ethical with good assessment/organizational abilities; and Prior EMR documentation skills.


PHYSICAL DEMAND


Able to perform tasks involving physical activity which includes lifting, bending and standing; and Able to hear, read and understand written and verbal communications.
