The Nurse Practitioner works collaboratively with the health care team. He/she will assess, plan, implement and evaluate care for patients. He/she will, in collaboration with other team members provide programs, treatment and counseling appropriate to the patient's plan of care. He/she will consistently demonstrate proficiency in working with the designated patient population, and serve as a unit resource and teacher.
- 1. Accountable for the delivery of quality patient care utilizing a therapeutic process.
- 1.1 Conducts individual exams, evaluation and treatment to patients.
- 2. Evaluates outcomes of patient care
- 2.1 Identifies problem prone practices and initiates solutions to maximize desired outcomes.
- 3. Acts as a role model
- 4. Ensures that staff is familiar with current theories and practices.
- 4.1 Identifies gaps in knowledge and barriers to practice. Works collaboratively to plan programs designed to update staff.
- 5. Demonstrates Leadership Skills
- 5.1 Works collaboratively with the Medical Director to establish an efficient and patient friendly environment.
- 6. Responsible for promoting the Greenwich Hospital Patient/Guest Relations program.
- 6.1 Assures that all patients are treated with respect and that needs are met promptly.
- 7. The Nurse Practitioner must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patients requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/department's policies
MSN Required, Graduate of Nurse Practitioner Program. Current CT APRN License.
3-5 years experience in area of specialty preferred.
Licensed as a NP, Certification in area of specialty; Basic Life Support (BLS), NRP
May be exposure to infectious diseases. Moderate stress from providing care and assisting with programs.
