The Oncology Nurse Practitioner/APRN is an advanced practice nurse who is an integral part of a multidisciplinary disease team providing specialized care to a population of patients with Cancer. The incumbent works in
collaboration with the attending physicians and other members of the disease team to manage patients during all phases of the disease process from diagnosis, through treatment and discharge or end of life. These positions exist in both inpatient and outpatient settings.
Responsibilities:1. Clinical Practice
1.1. Assumes direct patient care responsibilities for a group of patients as directed by the Disease team leader/attending physician.
1.2. Identifies and implements medical plan of care in collaboration with attending provider.
1.3. Performs physical assessment and other direct care procedures, including bone marrow aspirations and biopsies, if credentialed/privileged to do so.
1.4. Places orders in the CPOE system; writes chemotherapy/biotherapy orders, if credentialed to do so, following the plan of care established by the attending physician
1.5. Monitors patients for progress and toleration of treatment.
1.6. If inpatient based, ensures that a plan of care is established for discharge to a safe setting; is responsible to perform appropriate duties related to discharge in a efficient fashion.
1.7. Documents patient assessment and interventions in medical record, following YNHH policies and procedures
1.8. Evaluates patients who have urgent needs in Infusion Center or on patient care units; communicates to attending physician.
1.9. Assists attending physician in identifying candidates for clinical trials; follows patients on study as per protocol.
2.1. Partners with all members of the health care team to ensure coordinated communication to patients and families.
2.2. Readily consults with other members of the health care team, including social work, religious ministries and other team members to ensure that patients are receiving supportive care services consistent with the plan of care.
2.3. Responds to patient and family telephone calls received by the disease team; documents the nature and disposition of those telephone calls in the medical record.
2.4. Ensures communication with referring and primary physicians as to the progress of the patients under his/her care.
2.5. Participates in disease team meetings and Multidisciplinary patient care conferences.
2.6. On the inpatient unit, rounds with the team and nursing staff; ensures that nursing staff have the necessary orders to support treatment.
3.1. Serves as a role model for nursing staff; provides inservice education, grand rounds, etc to members of the team.
3.2. Upon request, serves as a preceptor for APRN students at participating educational institutions.
3.3. Collaborates with faculty from YSN on projects or serves as a clinical resource.
4. Professional Development
4.1. Ensures that he/she attends sufficient educational opportunities to stay current in his/her field.
4.2. Maintains certification in appropriate specialty
4.3. Participates in a professional organization.
4.4. Meets all requirements for mandatory education as defined by YNHH
Graduate of an approved PA or APRN program required. Graduation from an APRN position with concentrated oncology clinicals is preferred.
Experience as an APRN preferred. Oncology RN experience is preferred.PA experience is preferred
Appropriate national certification by either AACN or ONS. Current licensure to practice in the State of CT. Either CT APRN Or PA License
Excellent clinical skills and judgement; possesses analytical skills to evaluate care needs; excellent interpersonal skills and relationships with patients, peers and physicians. Proficient in computer skills.
