SUMMARY

Reports to the NEMG Quality and Safety Medical Director with matrixed reporting to ACO/Value Based Program Executive Director.  ACO/Value Based Business Analyst and Risk Management matrixed reporting into the Quality & Safety Manager.  Quality & Safety Manager direct reports include Patient Experience Coordinator and Consultants.

 

The incumbent will provide strategic direction and operational oversight for quality measurement and improvement, patient safety and patient experience in NEMG.

Responsibilities

RESPONSIBILITIES

 

  1. Works with Quality and Safety Medical Director to define quality and safety vision and programs within NEMG.
  2. Key leader in defining, measuring and reporting on quality across NEMG. Defines these with the Quality and Safety Medical Director. Works with NEMG Quality and Safety Medical Director and System Clinical Integration team to develop, implement and track the quality improvement program. This will include but not be limited to development of Clinical & Evidence-based Guidelines.
  3. Works with programming staff to develop, maintain and verify key dashboards for Quality Value Safety (QVS) and the Clinician Quality Bonus Program. Responsible for the data related to the Clinician Quality Bonus Program. Provides oversight to the process and metric development, staying within the approved timelines. This program roles up to the Clinical Compensation Advisory Council (CCAC) and is coordinated with the Quality and Safety Medical Director on agendas and getting approval for the metrics.
  4. Collaborates with YNHHS Quality and Safety teams to improve quality and safety across the YNHHS system. Attends system meetings on quality and safety, representing NEMG reporting back to the Quality and Safety Medical Director.
  5. Drives NEMG Patient Experience strategy in collaboration with YNHHS setting service excellence standards and developing measures that impact NEMG’s bottom line.
  6. Requests data pulls on relevant quality and safety measures and metrics from NEMG data team.
  7. Identifies opportunities for improvement in quality and safety.
  8. Works with data and from within Epic to identify barriers to quality and safety improvement.
  9. Develops action plans for approval from the Quality and Safety Medical Director. Works with nursing staff and clinicians on approaches to improve quality and safety to develop action plans.
  10. Interacts and educates clinicians and staff on action plans designed to improve quality and safety. Monitors progress and reports back to senior leadership in regular intervals.
  11. Collaborates with EPIC optimization teams to improve clinician workflows designed to improve quality and safety.
  12. Prepares materials for the Quality Value and Safety Committee (QVSC) together with the Quality and Safety Medical Director and Executive Director of Value Based Purchasing.
  13. Acts as key liaison with value-based team to improve overall quality results as measured by achieving quality targets in defined measures.
  14. Collaborates with NEMG ACO and Value-based team on measuring key quality measures annually. Works with ACO Quality Program Manager in developing strategies to achieve targets.
  15. Maintains established safety, environmental and infection control standards. Takes appropriate corrective action for identified infractions.

Qualifications

EDUCATION (number of years and type required to perform the position duties):

Bachelor's Degree in Nursing, Public Health, Health Care Administration, or other health related field

required. Master’s degree preferred.  Degrees in Public Health strongly preferred.

 

EXPERIENCE (number of years and type required to meet an acceptable level of performance):

Minimum of 5-7 years of full time clinical experience, preferably in an ambulatory setting; some inpatient experience helpful; experience on the payor side a plus. Experience working directly with physicians strongly preferred.

 

SPECIAL SKILLS:

Excellent interpersonal, communication organizational and leadership skills. Ability to work collaboratively and independently. Demonstrated maturity and good judgment. Expert knowledge of quality improvement methods, processes and practices relating to patient care and clinical outcomes. Demonstrated knowledge of quantitative and qualitative data management including but not limited to entry, analysis, interpretation, validation, reliability. Demonstrated proficiency in Microsoft Office software (Word, Excel, PowerPoint and Outlook) and YNHHS information systems and external databases.

 

ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, or other areas of accountability):

Responsible for directing the planning, development, coordination, implementation and evaluation of the multiple facets of Quality and Safety in coordination with the Quality and Safety Medical Director.

 

COMPLEXITY (describe planning, problem solving, decision making, creative activity, or other special factors inherent in the responsibilities of this position):

Advises in strategic planning, priority setting, monitoring and evaluation methods; demonstrates in -depth knowledge of the healthcare quality landscape, patient care and accreditation standards

 

LICENSURE/CERTIFICATION:

Lean and/or six sigma trained. RN licensure preferred but not required.
