SUMMARY

As a core member of the Care delivery team, the Clinical Nurse practices as the patients nurse and collaborates with the team to facilitate the coordination of patient care at the unit level. Through utilization of the nursing process, the clinical nurse diagnoses and treats patient and family responses to actual or potential health problems. The Clinical Nurse is accountable to the patient, family, peers, and Manager.

Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based on the 4 principles of PFCC: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence by creating a great "First Impression" by demonstrating exemplary customer service skills for all customer groups including patients, families/friends, physicians, staff and support department personnel. Responsibilities

  • 1. Administrative Team Manager
    • 1.1 Manages administrative operations of the program by directing front desk services and administrative support team. Directs staff regarding daily schedules and assignments and implements change as necessary. Manages overtime costs when appropriate
  • 2. Breast Center Outreach
    • 2.1 Responsible for maintaining current and developing new professional relationships with Breast Cancer Advocacy Groups and Community Health Organizations.
  • 3. Patient/ Customer Satisfaction
    • 3.1 Serves as the point person to triage and handle Breast Center patient complaints. Works with staff and physicians to address complaints in a timely manner.
  • 4. Quality Improvement:
    • 4.1 Works with leadership to develop and monitor performance/quality data for all Breast Center services on a quarterly basis
  • 5. Patient Follow Up
    • 5.1 Responsible for reviewing disease team no show reports and facilitating reminder and follow up letters to patients and referrals sources in a timely manner
  • 6. Breast Center Program Development and Operational Support
    • 6.1 Responsible for promoting the Breast Center to all customers by acting as a loyal, supportive, and informed spokesperson of the program, the physician practice, and the Hospital.

Qualifications

EDUCATION

 

Baccalaureate Degree required with Baccaluareate degree in nursing or related allied health profession preferred. Masters Degree preferred.

 

EXPERIENCE

 

Two to three years experience in a leadership/supervisory position required.

 

SPECIAL SKILLS

 

Demonstrated ability to manage staff and promote engagement and development. Able to develop and implement a multiyear plan that maintains the competitive service for the Breast Center. Demonstrated ability to collorborate with corporations and community groups. Demonstrated ability to manage multiple priorities. Excellent communication skills. Knowledge of and proficient in the operation and use of computer-based information systems, to facilitate all aspects of care delivery. Knowledgeable in budgeting and financing. knowwledgeable of Quality Improvement processes, and capable of implementing concepts. Demonstrated ability to implement and support change required. Demonstrated ability to interact with patients and families as well as community.

 

PHYSICAL DEMAND

 

Twenty four hour a day accountability for patient follow up and communication. Exposure to biohazards blood and body fluids. Must be knowledgeable of hospital and YMG policies and practices.

 

Additional Information


Management experience preferred
Working knowledge of clinical secretary, physician office liaison, and intake preferred
Oncology and.or breast knowledge experience preferred
