Coach patients in their transition from hospital or medical facility to home health care. The Transitions Nurse reports to Quality Manager. HIPAA Access Level 4.

Responsibilities

  • 1. Interacts with Case Managers and Discharge Planners at area facilities to determine patients appropriate for Transitions Coaching.
  • 2. Consults with VNASC staff for appropriate referrals for Transitions Coaching.
  • 3. Meets with patients before discharge to coach them through their transition to home health care. Makes follow-up telephone calls as needed to continue the coaching process and to coordinate with the VNASC staff.
  • 4. Acts as a resource for L&M Hospital referrals including making assessments for appropriateness for home health care.
  • 5. Assumes responsibility to participate in at least 12 hours of education per year.
  • 6. Performs other duties as assigned by the President/CEO.
  • 7. Projects a positive attitude and enthusiasm about agency mission, philosophy, and policies.

Qualifications

EDUCATION


B.S.N. (preferred) or R.N., current Connecticut license, one year recent experience in community health nursing with home visiting. Knowledgeable in coaching strategies. Satisfactory work references.


SPECIAL SKILLS


Medium to heavy. Constant handling of under 10 pounds to occasional handling of 70 pounds. Frequent driving required. Frequent use of computer. Standing walking, and handling are performed on a constant basis. Sitting, carrying, bending, lifting/lowering, pushing/pulling with whole body and with arms are performed on a frequent basis. Squatting/kneeling, twisting, and reaching are performed on an occasional basis. The highest physical demands of this position are incurred while transferring a client requiring maximal assist of one or more persons, for example, side lying to sitting, sitting to standing, and lifting from chair, commode, toilet, or bed. Loads lifted or forces exerted up to 70 pounds for client weighing up to 220 pounds in height range of 15-36 inches. Clear vision required. Accurate hearing required.
