Under the direction of the Manager, Clinical Integration this position has responsibility and accountability for successfully developing, implementing, and executing front line operations for clinical integration and population health programs across YNHHS and the wider network. The role involves substantial collaboration with leaders and internal and external stakeholders to facilitate the coordination and delivery of healthcare services, to develop and implement ambulatory quality improvement and practice transformation programs, and to support the transition to value based care to enhance the health of the communities we serve. This position requires the ability to seamlessly and independently manage projects as directed by the Manager and Director, Clinical Integration & Population Health.

Responsibilities

  • 1. Develops, implements and executes strategies and programs that advance the organization?s development of a clinically integrated network.
    • 1.1 Supports the physician-led governance and committee infrastructure for clinical integration in conjunction with clinical and administrative leaders
  • 2. Develops programs and infrastructure that support population health management.
    • 2.1 Coordinates development of population based registries, clinical practice guidelines, and evidence based ambulatory quality measures with key stakeholders in the clinical and information technology departments, aimed at establishing, measuring and monitoring a high standard of clinical excellence for our patients.

Qualifications

EDUCATION


Bachelor's degree in business, public health, nursing, integrative health, healthcare administration or other related field required. Masters degree preferred. Clinical background is welcome. Advanced training in project management, Lean Six Sigma, performance improvement and quality improvement methodologies is strongly preferred.


EXPERIENCE


5-7 years of progressive experience required with increasing responsibilities and experience in managing programs with demonstrated successful outcomes. Experience working with clinical integration, quality improvement, and value based healthcare programs strongly preferred.

SPECIAL SKILLS


A passion for improving the delivery of health care in the ambulatory setting and across the continuum of care. Superior written and verbal communication skills, ability to develop effective communication materials and presentations and communicate across diverse stakeholders. An ability to work collaboratively with and influence individuals at varying levels within the organization. Past experience delivering results in a matrix work environment. Must be flexible, adaptable, team-oriented, and able to work independently and autonomously, a critical and strategic thinker and excited for a steep learning curve.


PHYSICAL DEMAND


The office is located in New Haven, CT however the position requires the ability to travel to other locations across the health system from time to time. In addition, the candidate must be able to attend early morning meetings from time to time.
