The Nurse Practitioner provides comprehensive care to patients under the supervision of the advanced heart failure attending physicians and as directed by established protocols. In collaboration with the health care team, the Nurse Practitioner provides continuity of care to the target patient population and contributes to the education of patients, their families and other health care professionals as necessary.

Responsibilities

• 1. Clinical Practice
• 1.1 Delineation of Activities: function as a collaborating provider in the ambulatory heart failure clinic, engage in activities to promote programmatic growth, build and maintain relationships with referring providers and support the vision of the program directors.
• 2. Performs initial and ongoing evaluation of all patients, including history and physical exam, in the outpatient setting.
• 3. Performs special procedures, as indicated, in accordance with approved protocols.
• 4. Will be able to direct or assist in the care of patients requiring hospital admission.
• 5. Evaluates patients for additional intervention, particularly those recently discharged from the hospital.
• 6. Under the family centered care model, provides care that supports patient/family collaboration.
• 7. Educational Efforts: Identify learning needs and support nursing, medical staff, patients and their families. Provide bedside teaching and supervision of procedures. Participates in continuing education.
• 8. Research: Participate in research as deemed necessary by advanced heart failure attending physician.
• 8.1 The practitioner may participate, collaborate, or initiate research activities as appropriate and consistent with the goals of the advanced heart failure center.
• 9. Professional Development:
• 9.1 Participates in at least one (1) professional organization.
• 10. Evaluation: Will be done at six month probationary period and annually with input from the advanced heart failure attending physicians.

Qualifications

EDUCATION

MS or MSN. Graduate of an approved Nurse Practitioner program. Current Connecticut APRN license (or eligible).

EXPERIENCE

APRN or PA with several years of experience caring for patients with heart failure or working in transplantation. Job will entail care primarily in the ambulatory setting of the patient with heart failure under evaluation or awaiting heart transplantation as well as the patient post heart transplantation.

LICENSURE

Must have a CT nursing license and current CT APRN license

SPECIAL SKILLS

Excellent basic medical knowledge with clinical skills and judgement. Possess analytical skills to evaluate care needs, ability to gather relevant clinical data, perform thorough physical examinations, and possess excellent interpersonal skills, communication, and leadership abilities and exhibit good judgement dealing with parents, families, and allied health professionals. Computer skills necessary.

PHYSICAL DEMAND

May require standing for long periods.

Additional Information

Prior experience in Heart Failure/Cardiology is required. Position is based in New London. Occasional travel to other Disease Management clinics may be required.
