The Nurse Practitioner provides comprehensive care to patients under the supervision of the advanced heart failure attending physicians and as directed by established protocols. In collaboration with the health care team, the Nurse Practitioner provides continuity of care to the target patient population and contributes to the education of patients, their families and other health care professionals as necessary.

Qualifications
EDUCATION
MS or MSN. Graduate of an approved Nurse Practitioner program. Current Connecticut APRN license (or eligible).
EXPERIENCE
APRN or PA with several years of experience caring for patients with heart failure or working in transplantation. Job will entail care primarily in the ambulatory setting of the patient with heart failure under evaluation or awaiting heart transplantation as well as the patient post heart transplantation.
LICENSURE
Must have a CT nursing license and current CT APRN license
SPECIAL SKILLS
Excellent basic medical knowledge with clinical skills and judgement. Possess analytical skills to evaluate care needs, ability to gather relevant clinical data, perform thorough physical examinations, and possess excellent interpersonal skills, communication, and leadership abilities and exhibit good judgement dealing with parents, families, and allied health professionals. Computer skills necessary.
PHYSICAL DEMAND
May require standing for long periods.
Additional Information
Prior experience in Heart Failure/Cardiology is required. Position is based in New London. Occasional travel to other Disease Management clinics may be required.
