The Geriatric Clinical Manager is responsible for ensuring high quality, safe, reliable, and efficient care, in the Adler Geriatric Center. The Geriatric Clinical Manager uses managerial, interpersonal, and clinical skills to model, motivate, and monitor performance resulting in superior quality service, and financial outcomes. Responsibilities and accountabilities are held on a 24/7 basis. The Geriatric Clinical Manager will collaborate with department administration on a consistent basis to ensure communication of effectiveness of the above and all role responsibilities defined below. Leads and coordinates the patient/family evaluation and the formulation and implementation of a treatment plan. Collaborates with the physician and patient care associate caring for each patient in the evaluation and development of a treatment plan with input from other professions depending upon the presenting problem. Assumes primary responsibility for monitoring outcomes of collaborative interventions. Responds to the common issues raised by older persons with cognitive and physical impairments. Reviews the patient's problems while providing attention to the cost and burden upon the patient and family. Maintains a delicate balance between the patient's medical and nursing needs and the care needs of patients and families.
Master's degree in nursing required.
5 years of clinical nurse experience and a minimum of 2 years of management experience to include human resources, finance, and patient experience. Clinical experience with geriatric patients and their families as well as medications preferred.
Current CT state RN licensure.
Ability to direct a diverse work force in an ambulatory setting. Passion for leading others in a challenging and changing environment by utilizing innovation, technology, and best practices. Knowledge and experience in assessing, analyzing and formulating complex clinical, social, financial and psychological issues as they pertain to older persons and their families. Knowledge of psychopharmacology preferred. Extensive decision making responsibility and the demonstrated ability to independently manage complex geriatric clinical care preferred. The case manager must be skilled in organizing and prioritizing data and information.
MSN with 5 years clinical nurse experience and 2 years management experience to include human resources, finance, and patient experience in Healthcare setting.
